1. Program Description and Logic Model Narrative (2–3 pages)

Program Context

· Program identification: Name, location, and sponsoring agency

· Mission and goals: Clear statement of program purpose

· Target population: Specific demographics, selection criteria, and size

· Context: Community setting, relevant statistics, and local conditions

· Problem/need addressed: Evidence-based description of the issue (cite at least 3 sources)

Theory of Change and Logic Model Explanation

· Theory of change: Clearly articulate the program’s theory of change, that is, how and why the program activities are expected to lead to the desired outcomes. Explain the causal mechanisms linking program components to expected results with theoretical basis and research support. It is essential to cite scholarly sources to support this section.

· Assumptions: Underlying beliefs about why the program should work

2. Logic Model Diagram (1 page)

Create a professional logic model that includes:

· Inputs/Resources: Staff, funding, facilities, partnerships, materials

· Activities: Specific program components and interventions

· Outputs: Quantifiable products of activities (e.g., # of sessions, participants served)

· Short-term outcomes (0–6 months): Immediate changes in knowledge, skills, attitudes

· Medium-term outcomes (6–18 months): Behavioral changes, skill application

· Long-term outcomes/Impact (18+ months): Sustained changes, community impact
