Frankie is a 22-month-old boy with the following medical history: He was born full-term. Parents report a history of spitting up since birth. This worsened over time, and he had vomiting multiple times per day for the first 6 months of life. He was diagnosed with gastroesophageal reflux and placed on reflux medication at 6 months of age. He is currently followed by a gastroenterologist. Medication reduced the frequency of vomiting, but he continued to show signs of discomfort with feeding. Respiratory history is unremarkable. Growth and weight gain have been adequate. Parents are concerned for speech development. He currently has six words in his vocabulary. His parents also report sensitivity to loud noises and difficulty with transitions from one activity to the next. He is not currently enrolled in any therapy services.
