Reverend Foster is a patient in your primary care office. You know him/her to be a respected member of the community who is an outspoken anti-drug advocate. He/she is married and coaches his/her 16-year-old daughter’s softball teams. You are seeing him/her today for follow up of an urgent care visit 2 days ago. You have not seen him/her since before his/her knee replacement surgery 8 weeks ago. The note from urgent care indicates that he was seen for pain and insomnia. He reported to them that he had stopped his PT early as he/she felt he no longer needed it, and that the orthopedic doctor discontinued his oxycodone as the replacement appears to be healing well. There was no evidence of problems with the knee replacement, but given his complaint of severe and persistent pain, he/she was given a new prescription by the urgent care provider for oxycodone twice a day for five days until he/she can be re-evaluated by his/her primary care provider. It was also noted by the urgent care provider that he/she was positive for muscle aches, anxiety, and intense feelings of restlessness upon stopping the oxycodone, indicating a possible dependence.
