Share an outline of Section V (Evaluation Plan) of your Change Proposal with your peers. You will collaborate with a small group of your classmates to share ideas and offer feedback and suggestions to one another in an informal setting. This collaboration within your group will assist you in further developing your Change Proposal to be submitted for feedback from your instructor next week.

In your evaluation plan post, be sure to cover the following:

Propose steps for evaluating your suggested changes based on your change theory and evidence on existing best practices. Include the indicators that you will use to measure success, both in implementing the change itself and in ensuring quality outcomes for diverse patients.
Identify appropriate benchmarks from which to measure the effectiveness of the change.
Discuss the processes you will use for gathering feedback from relevant stakeholders—including nurses, patients, and their families—on what is and is not working in order to support continuous improvement.

In your small group discussions, you are going to share your evaluation plan outline.

In your feedback, you want to give suggestions to at least 2 of your peers about their plan. Are they missing anything critical? Are there any outcomes they are missing? How would you recommend gathering the data or doing a survey? Do you have something that worked great in your organization around their topic? This is the change to share to help others figure out how they would make their change project work!

Note to writer: This order is for an outline for section V for a change project proposal we are working on. I will attach the change proposal document and also rubric to see how it is categorized. So far we have done section I-III, I will place a separate order for the section IV of the change proposal. So this outline is the next section of the project which will be due next week. i will also attach a sample from the instructor, which is based on CLABSI. For this outline it is supposed to be done in point form to make it precise and easy for my peers to read.
