In the state of California, the RN who obtains an NP license does not have an additional scope of practice beyond the RN scope of practice and must follow standardized procedures for more guidance on how they are expected to perform specific job duties (BRN, 2011). Standardized procedures allow the NP to practice procedures that are considered beyond the scope of RN job functions and without these procedures, NP’s would be very vulnerable to legal action. Two safeguards of utilizing standardized procedures includes providing strict guidelines on how specific procedures should be handled by the NP as well as support if legal action were to be taken against the NP, as long as the APRN followed this standardized procedure well within their workplace (Hayes, Wolf, Labbe, Peterson & Murray, 2017). In addition, these standardized procedures must include information regarding the furnishing of drugs and devices so described by the supervising physician and/or surgeon in order to be compliant with the Board of Registered Nursing’s amended chapter 914 in 2000 (BRN, 2004). Supervising physicians, though not always physically present, are there to support the nurse practitioner in their work. Three scenarios in which the NP may want to consult the attending provider include a complicated physical presentation in which a work-up ruled out multiple differential diagnoses, a patient returning with similar symptoms in which an antibiotic was prescribed and consultation is needed to decide how much additional medication or method is required to treat the cause, and when all options are exhausted in the setting and the patient may need to be referred to a specialist.
