The Risks of Collecting Data on Human Populations: Privacy, Security, and Ethics in Population Health Informatics
Identity theft is responsible for the loss of billions of dollars every year (Siciliano, 2017). Most people are familiar with overt types of identity theft, such as financial, criminal, and Social Security. In the healthcare field, medical identity theft occurs when someone uses identifying information such as insurance information to obtain healthcare goods and services (World Privacy Forum, n.d.). These types of privacy and security breaches are typically premeditated and conducted with criminal intent. In healthcare organizations, however, the very same technology that provides so many benefits in terms of information sharing and storage of data can pose high privacy, security, and ethical risks to healthcare professionals with even the best of intentions. This is why it is so important to be familiar with risks in these areas as well as strategies to prevent breaches of sensitive health information. One such strategy is to follow ethical principles as outlined by such organizations as the National Institutes of Health and understand when and how violations have occurred. Another way to take a proactive stance to protecting sensitive data is to practice Privacy by Design, or Pb.D.

Yet, for the individuals who have these diseases and conditions as well as their communities, states, and even countries at large, the actual costs of certain chronic conditions and diseases tend to mount, and can drastically reduce savings and coffers. Once individuals get to the point where they can no longer pay for their own care, it often falls to governments and other entities to make up the difference.
Governments and private organizations of all types, then, have a stake in managing the health of populations in order to reduce costs and improve measurable outcomes to these populations. Informatics tools are critical to identifying these populations and providing interventions in many cases. Health informatics tools are also critical to support the ongoing data analysis that occurs when health and demographic data are continuously collected on populations. Similar to the goals of prevention and wellness, it is important to consider the three levels of prevention—primary, secondary, and tertiary—in developing population health informatics strategies related to disease management and chronic care.

Health Informatics and Environmental Issues
For the purposes of this course, the term environmental refers to a broad set of factors that intermingle with the health of populations. Some environmental issues such as unsafe drinking water supplies or issues with sewage or air quality make local and national headlines. The health and wellbeing of populations may be improved by proper nutrition; clean air; clean drinking water; and clean, disease-free housing. These are all factors that affect the health of many different populations.
More insidious factors affect the overall health and wellbeing of populations in both developed and developing countries, as well. For example, as discussed by Ahangari, Atkinson, & Garrick (n.d.), factors such as the degree to which individuals and entire communities are dependent upon automobile transportation for daily functions serves as one of the primary barriers to reaching a zero-fatality safety vision in the design and use of roadways. Therefore, when considering the health of populations, we must also think of infrastructure of all types within the communities in which we live and work, not just water, air, and food.

Different nations around the world face different challenges related to the health of populations. Population health informatics solutions also very widely in their design and effectiveness depending upon how nations define goals and expectations for technical interventions. Business Proposal on Data Quality and Governance
What does success mean in terms of data management? How do you know when you are using quality data for health informatics strategies? The answers to these questions lie in knowing the difference between data, information, and knowledge.
Put simply, data are facts, measurements, or characteristics of an entity or object. Used in the health field, they do not tell the whole story of a disease or condition without context. For example, demographics and physiologic measures such as blood pressure, weight, or blood glucose levels are examples of data. When data is put in its context, it becomes information. Nurses and physicians may use systems and processes to aggregate consumers’ demographics, and physiologic measures may be used in reports or mobile applications to follow up on treatment. These are examples of creating information from data. Once this information can be used to analyze the “why” and “how to” behind the findings, it becomes knowledge. In our example, if a nurse practitioner uses information in a consumer health report to improve the time and availability of hemoglobin A1c blood testing for consumers at risk of developing prediabetes, we move from the information stage into knowledge.
Even when health professionals using a health informatics strategy have successfully turned data into information and gained knowledge, it is not always clear when success has been achieved. For one thing, what kind of goal was set—financial success, clinical success, or perhaps both? Attaining both of these goals for all entities may be desirable, but not realistic. The data that inform these decisions related to quality care often reside in disparate electronic health record systems and databases. As chronically ill patients consume more healthcare services and visit different specialized providers, the amount and complexity of data as well as the tasks involved in managing to achieve financial and clinical success increase exponentially.
As evolving leaders in the field, it is your responsibility to work with colleagues to define success by way of establishing and leading organizations toward meeting those goals. The effective use of health informatics solutions is one of key success factors in achieving this.

Capital Operating Budget Using Proforma Analysis
Information technology-based initiatives are significant and strategic investments for organizations. Costs must be considered, such as upfront items, incurred early in the system development life cycle, and ongoing costs, which are incurred on a perpetual basis. These costs must be actively estimated during the project/program planning process and updated on a regular basis. These estimates inform the organizational budget process and must also be incorporated into return-on-investment (ROI) analysis. The concept of outlining an inclusive set of costs for an IT investment is called the total cost of ownership (TCO) for the technology asset.
One of the most efficient methods to execute this concept is through the development of a multi-year proforma, which is updated on an annual basis. A proforma is not a budget; it is a long-term estimate of the capital and operational costs for the initiative. A proforma should be carefully updated as needed and used to inform both the budget process and the ROI in the organization. These updates should take place on an annual basis, at a minimum. Technology-based initiatives are complex investments for any organization, and communication between project/program planning staff, financial staff, and operational leadership is imperative.
This week, you explore how to develop a capital operating budget using proforma analyses. Using a multi-year proforma template and explanation material provided, you use a tool, a 5-year informatics cost proforma, for the pilot launch of a patient portal in a PCP office in which PCPs are working to effectively manage the health of chronically ill populations, such as persons with diabetes, heart failure, asthma, and anxiety/depression.
Remember that the proforma represents estimates of costs, which is one side of the effectiveness equation. The other side of the equation is represented by the tangible benefits enabled by investments in informatics. Health leaders have budgetary responsibilities to not only investigate and implement solutions, but to ensure that there are business models in place to allow those solutions to be sustainable. In choosing an economic model to serve as a framework for business decisions, they must consider the capital and operational costs of designing and implementing the health informatics solution.
Referring to your Final Project topic chosen in Milestone 1, investigate the appropriate economic model in order to propose a health informatics solution for your chosen topic.

For this Assignment, you will extract datasets from the data sources provided in Weeks 5, 6, and 7 that relate to your chosen Final Project topical area. Using datasets that you download from the data sources, you conduct basic statistical analyses such as observations regarding mean, median, and mode seen in the data in order to inform decisions related to health informatics solutions.
Remember that in addition to providing a 1-page description of your Final Project in this milestone defining the population and related health challenges, you also include a relevant chart or graph that summarizes your dataset and its relationship to your Final Project. Be sure that your graphic representation tells the complete picture of the story that you are describing in this project milestone.
To Prepare:
Review data sources that apply to your Final Project topical area from Weeks 5–7. Explore the datasets by sorting data and performing some basic analysis such as examining total counts, demographics, and quantitative topics measured in the dataset, as well as basic descriptive statistics such as averages and means in the data.
Using Microsoft Excel tools, sort the data and graph the data, looking for trends and patterns that you see in the datasets that you extract from each data source. What are the data telling you about the populations involved? How do the data that you see in the datasets compare to quantitative information and other studies that you are reading in your peer-reviewed research on your topical area?
