Discussion 1 Compare the Use of Cognitive Behavioral Therapy (CBT) Across Individual, Family, and Group Settings. CBT is one of the psychosocial therapies, which has been proved to be effective in treating variety of mental health disorders like depression and anxiety disorders (Paul, 2024). Therapist can utilize CBT methods such as thought reframing to change the way of thinking which can improve the mood and behaviors (American Psychiatric Association, 2023). There are multiple therapy modalities for CBT treatment, which include individual therapy, family therapy, and group therapy. Individual therapy only involved the patient and the therapist. During the family therapy session, both the family members and the patient along with the therapist are included in the treatment session. The group therapy is the session was held in a group form, which include a group of patients with similar mental health problems or different mental health issues. In the clinical site that I am doing practicum this quarter, which is an inpatient psych unit. the patient is encouraged participate all the sessions, including individual, group, and family sessions. Each sessions serviced different purposes. And the topic for each session can vary day by day. Analyze Challenges of applying CBT Approaches and Techniques Across Therapy Modalities Psychotherapy may be conducted in the forms of individual, family, couple, or groups. When applying CBT approaches and techniques during different sessions, there are some challenges sometimes. When doing group therapy for patients with similar issues such as suicidal ideation (SI). When group members were sharing their own stories, other patients may be triggered by the SI and lead to worsening and more intrusive SI or self-harm. But it proved to be helpful when utilizing CBT group therapy in treating patients with somatic symptom disorder (Jonsma, 2023). For the individual therapy session, the patient and the therapist have to establish a trustworthy relationship before the patient was willing to share the thoughts and stories. For family therapy session, if the family member and the therapist can reach an agreement regarding the treatment goal before they join the patient, then it will be easier for the therapist to maintain the boundaries and the paste during the treatment session. Recommend Effective Strategies for Overcoming Challenges in Applying CBT There are reports proved that individual CBT therapy is more effective than group CBT therapy in treating peer problems and social anxiety issues in children (Melero, 2021). But this is not a set answer for all the other mental health issues or for all the age groups. It might more depend on the patient’s preference and the patient’s mental health issues. For example, for a patient with depression and anxiety and the main stressors are from the family member, then the family therapy session might be more important. During the family session, the patient can openly communicate with the family member with the present and involvement of the therapist. In this situation, family therapy may be very helpful compared to individual or group therapy. But by doing individual therapy with the therapist, the individual may gain more knowledge and understanding the issues and how to manage them better in the future. During the group therapy, the patient can also understand other patients also have some similar issues or some more serious problems. By participating the group session, the patient may learn new skills from the therapist or peers. Each modality can serve different purposes and the patient don’t have to choose one from the others. It leads to a significant improvement in the patient’s quality of life and functionality. It is more effective than any other form of psychiatric therapy. It works on several principles, for instance, mental problems are based on a learned pattern of unhelpful behaviours. It also asserts that mental disorder patients can learn improved ways of living with the disorders, hence being able to relive their symptoms and being even more effective in their lives. It can be used in groups, family, and individual settings. In a family and group setting, CBT is an effective way of addressing mental illnesses that affect the whole family. In this case, it focuses on the different family members’ behaviours, emotions, and thoughts. Upon identifying a negative change in the different attributes, cognitive behavioural family therapy (CBFT) will enhance communication, reduce conflicts, and improve mental health outcomes (Reuman et al., 2021). Secondly, it emphasizes the need for recognition to shape the behaviours and emotions of the different family members. The family members are taught different ways of communicating and thinking through group sessions, which may include homework assignments to enhance new behaviours and skills. CBT, in an individual setting, works almost similarly to the group setting. However, in this case, CBT focuses on helping the patients to know their therapists. It includes homework exercises to aid the patients in learning new skills and behaviours (Alimoradi et al., 2022). OnOn an individual basis, CBT is carried out for 6 to 20 weeks, each session lasting for about 60 minutes. This may occur in a clinic, at home, or outside. The first session focuses on ensuring that the right therapy is administered and that one is comfortable with the entire process. Further sessions involve breaking down the problems into distinct parts until the goal of positive behaviour and emotional change is achieved. PMHNPs may face multiple challenges in the CBT administration in individual and family settings. One of the major challenges is dealing with different emotions from different people. People find it extremely difficult to change their emotions, so this calls for a more intensive therapy administration (Heidenreich et al., 2021). Secondly, it is very complex to administer primary care. For start-up patients, CBT may be so complex that they are unable to complete the required sessions.
