DNP 960 Topic 1 Individual Success Plan (ISP)
Planning is the key to successful completion of the Direct Practice Improvement (DPI) Project. The Individual Success Plan (ISP) assignment in this course requires your collaboration with your chair early on to establish a plan for successful completion of mutually identified and agreed upon specific deliverables for your programmatic requirements. Programmatic requirements are: (1) completion of required practice immersion hours, (2) completion of work associated with program competencies, and (3) work associated toward completion of your Direct Practice Improvement (DPI) Project.
General Requirements:
Use the following information to ensure successful completion of the assignment as it pertains to deliverables due in this course:
- Locate and download “Individual Success Plan” (ISP) document in the DC Network.
- Review the “DNP Program Milestones” document and identify which milestones apply to this course. Note: Not all courses have milestones.
- Determine what practice experiences you plan to seek in order to address each competency. Include how many hours you plan to set aside to meet your goals. Learners will apply concepts from each of their core courses to reflect upon, critically examine, and improve current practice and are required to integrate scholarly readings to develop case reports that demonstrate increasingly complex and proficient practice.
- Use the Individual Success Plan to develop a personal plan for completing your practice hours and how competencies will be met. Show all of the major milestones and deliverables.
- Within the Individual Success Plan, ensure you identify specific deliverables which can include the following: individualized DNP practice immersion contracts; comprehensive clinical log of hours applied to doctoral level learning outcomes; learner evaluations; mentor evaluations; current and updated CV; scholarly activities; GCU DNP competency self-assessment; reflective journal; course goals and plan for how competencies and practice immersion hours will be met; faculty and mentor approvals of course goals and documented practice immersion hours; and DPI Project milestones.
- Identify the specific deliverables you will complete throughout this course from those defined above or others negotiated with your faculty. You need to turn in a new Individual Success Plan for each DNP course.
- Identify the remaining deliverables you will complete in the upcoming courses.
- This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.
Consider the challenges you expect to encounter as you continue the practice hour and competency requirements throughout this course? How might you overcome these challenges?
You can renegotiate these deliverables with your faculty throughout this course and update your Individual Success plan accordingly.
Directions:
Complete the Contact Information table at the beginning of the ISP resource, and type in your signature and the date on which you completed the table.
Read the information in the ISP document including the following:
- Learner expectations.
- Derivation of the ISP.
- Instructions for completing the ISP.
Follow the instructions and complete the ISP.

DNP 960 Topic 1
Course Completion and Progression Acknowledgement
During DNP-960, you will be required to complete your IRB submission. You will need to have facility IRB approval (if your facility has an IRB) prior to submitting to GCU. If your project site does not have an IRB, you will be required to have site approval. GCU IRB submission is a mandatory achievement at this point in the program. If you do not submit to GCU IRB within the prescribed timeframe of the DNP-960 course, you will receive a zero for the Topic 3 assignment and you will not progress to DNP-965. Please utilize all of the IRB submission resources available in DC Network and iMedRIS (iRIS).
General Requirements:
- Confirm that your project meets the GCU DPI requirement; that it translates existing knowledge or guidelines into clinical practice to improve patient outcomes.
- Ensure that your clinical questions are feasible with your population and clinical site and in alignment with your method, design, data collection, and data analysis.
- Remove comments and track-changes from your proposal before submission to IRB.
- Accurate grammar, APA style, and scholarly writing is required. Please note that your chairperson is not an editor. You will be responsible for ensuring your compliance with APA.
- Remember that your manuscript will undergo several reviews from your chairperson, content expert, AQRs, department chairperson, and dean that will require multiple iterations. Please be open to feedback and incorporate the feedback into your manuscript to improve quality and rigor.
Looking ahead:
- In DNP-960 or DNP-965, after you receive GCU IRB approval, you can begin your project implementation. You should never be sitting idle while waiting for IRB approval. Begin writing your templates for Chapters 4 and 5 (without data until implementation with IRB approval). Your negotiated deliverables should include as much progress on these chapters as possible in the form of a working draft.
- In DNP-965, you will need to submit a final completed manuscript in Topic 5 to AQR. If you are unable to complete this submission within the designated timeframe, you will be required to take an extension course to finish your project or manuscript. In Topic 8, you will submit your final committee-approved manuscript for departmental review. You will have the opportunity for one iteration with the department until your manuscript is sent for Dean Review. If your manuscript is not deemed ready to be sent to the dean by the department, you will be required to take an extension course to continue to work with your chairperson. You will need to have a dean-approved manuscript, completed all courses with a passing grade, and 1,000 practice immersion hours to be eligible for graduation.
Directions:
Complete the following statement in a Word document and submit it to your instructor by the end of Topic 1.
I, (INSERT NAME), verify that I have read, understand, and will comply with all requirements set forth for course completion and progression. I have reviewed the DPI Project requirements and am fully aware of my options if I am not able to successfully complete the required level of milestone development.

Direct Practice Improvement (DPI) Project
Introduction
The Direct Practice Improvement (DPI) Project is a process that helps practice owners and managers improve their direct-care delivery. It can be used with any type of health care organization, from hospitals to home health agencies and urgent care centers. The DPI Project helps you identify opportunities for improvement in your practice, create goals for improvement and plans for achieving them, develop strategies for achieving those goals and plans, track progress toward those goals through regular meetings with your team members over time.
Direct Practice Improvement (DPI) Project
A DPI project is a process of identifying and improving the quality of your direct practice. The project team will work with you to identify areas where you can improve, and develop a plan for how to do so. You’re likely to be invited on this journey by one or more of these people:
-
Your Practice Improvement Facilitator (PIF)
-
A member of your Practice Improvement Committee (PIC)
This person will help guide your experience with the project, but they may not always be present during its execution. You may find that other team members are better able to answer specific questions about their roles in helping make improvements at your practice or organization as well!
Purpose
The purpose of the DPI project is to improve practice. In other words, we are working to make sure that your doctors are delivering high-quality care and getting it right so you can be happy with your visit.
The purpose of the DPI project is to improve patient care. We want our patients’ visits with you—the doctor—to be comfortable, efficient and painless! That means making sure that everything runs smoothly so no one has a bad experience at all when they come in for their appointment (or follow up).
What is the DPI Project?
The DPI project is a collaborative process for your practice, in which you and your staff identify areas for improvement. It’s an opportunity to improve the care that you provide to patients and team members, as well as yourself.
The DPI project will help you identify what works well at your practice, where there may be room for improvement, and how we can help each other achieve these goals.
Why do we do this project?
This project is designed to improve the quality of care, patient experience and efficiency in your practice. We do this by:
-
Improving the quality of your practice by implementing best practices to better serve patients.
-
Improving the efficiency of your practice through reduction or elimination of wasted time spent on administrative tasks that do not directly impact patient care. This can include things like paper work and record keeping (which would be completed digitally), as well as unnecessary phone calls between nurses and doctors about trivial matters that could easily be communicated via email instead; for example, “I forgot to put my name on this chart” or “I need another copy of this prescription filled today please!” These conversations take valuable time away from patients who really need us right now!
Who is involved in the project?
The project team is made up of you and your board. Other stakeholders are also involved, including:
-
Your staff
-
Your vendors (if you have them)
-
Other departments within your organization that have an interest in the project
The process of a DPI Project
A DPI Project is a collaborative process between your board and you. The DPI project is a process for improving your practice, and it will help you to identify areas of improvement and develop plans for making improvements.
The first step in the DPI process is to identify what activities need to be improved in order for your practice to provide excellent patient care. This can be done by looking at:
-
The work of other healthcare providers – What do they do that we don’t? How do they compare with our own practices? Are there any areas where we’re better or worse than others? What could we learn from them?
-
Patient feedback – Do patients like coming in because they know they’ll receive good care here? Is there anything else going on which might affect why this happens (like social media campaigns or new technology)? If so, how can we use those things as opportunities for growth instead of just problems
Your board and team will work with you to create goals and plans for your practice.
Your board and team will work with you to create goals and plans for your practice. These goals will be reviewed at regular intervals, including when making any changes to the plan.
You will also receive feedback on how well you are doing in meeting these goals, as well as suggestions for improvement if necessary.
If a goal isn’t met within a certain timeframe, then consider whether there is anything else that could be done differently or better to improve performance until that time comes around again!
Conclusion
Thank you for your interest in participating in the DPI Project. We look forward to working with you and your board to improve the quality of your practice.
