The purpose of this project is to allow you to become familiar with typical client files at the agencies and organizations in this field. It is necessary to understand the purpose of the documentation in files and to also to understand the relevancy of the documentation.
Consider that you work in ABM Mental Health and Addiction Services, I (your co-worker) come to work on a Monday morning, to find that you have been called away on a family emergency and are leaving the country.
I – (your co-worker) have been one of your clients to continue to work with. The client and you have been working together for almost six months, and have done a lot of work together. If you have kept accurate and complete files, I (your co-worker) should be able to read the file and continue to work with your client with little interruption in service.

There are a variety of documents that are available for your use. You may also find documents from the internet if you so choose. If you choose to use documents from the internet be sure that they are documents from credible sources.
When you are thinking about what you will need in your file, think about what documents would have been completed by you and your client from the first day you met up until the current date.

Make sure documents are in separate sections IE. Mental health assessments would not be in the middle of the progress reports

Profile :Drey is a 22 year old male. Drey has been involved with child welfare since his early teens. He has no family contact except for a foster family he lived with some time ago. He sees this foster family a few times a year for special occasions and holidays. As Drey is now 22 years old he no longer has any support services through Children’s Services. Drey would like to get some sort of secondary education, but he just hasn’t gotten around to it, mainly because he is unsure of what he would like to do. Drey currently works seasonal jobs. He makes more money in the summer, less in the winter, but he always has work due to the connections he has built. Drey shares an apartment with a friend who also has few family ties and was in the System. Drey uses cannabis on a daily basis for his depression and insomnia. He doesn’t feel he can get through a day without using. He does believe that he is using too much and is concerned about it. Many people have told him to see a counselor about his depression, insomnia, and cannabis use. Drey has seen an addiction counselor in the past when his social worker pressed him too, but that was a few years back. He had a good experience and so is open to seeing someone and makes himself an appointment. ( I need help with he progress note in this profile case and , updated ) thank you.
