Case Scenario: George Lopez is a 10-year-old Hispanic male who lives with his mother and two younger sisters (8 and 7) in a subsidized housing facility in Brownsville, Texas. All three children are U.S. citizens. George’s father, a US citizen, is currently in prison serving a 25-year sentence for felony drug trafficking (Cocaine). George was only one when his biological father went to prison and has no contact with him. George’s mother, Isabel, entered the United States illegally with her parents when she was 13. She became pregnant and gave birth to George when she was 15. Because of the shame she brought to her family by getting pregnant without being married, her parents refuse to have any contact with her. She was granted a marriage visa after she married the father of her two daughters, but he divorced her shortly after the second daughter was born. She never completed high school.
Isabel is currently unemployed and lives in subsidized housing. She receives supplemental nutrition assistance for the children through the SNAP program. She also receives additional financial assistance through other safety-net programs. Yvette likes to party and uses cocaine regularly. Though housing rules forbid this, she frequently has people over at all hours. Commonly, Yvette has men over who consume alcohol, cocaine, and occasionally other drugs. Cigarette smoke regularly fills the air, as does the residual smoke from crack cocaine. Not uncommonly, Isabel engages in sexual acts with various people in exchange for drugs and other favors (food, money, transportation, etc.). Needless to say, the children are regularly exposed to people engaging in illegal drug activity. Though Isabel tries to keep them separate from her drug use and sexual acticvity, only so much can be kept hidden under a small roof. In addition, the children sometimes miss meals, and get insufficient sleep.
George was recently referred to a school social worker because he seemed to be exhibiting developmentally inappropriate sexual behavior. According to the teacher, she observed George put his hand down the front of his pants while standing in front of the desk of a female classmate. With his hands down his pants, he made thrusting motions toward her until she let out a scream. She reported that the female classmate was appalled, shocked, and perhaps traumatized by George’s behavior. His teacher also reported that George frequently wears the same clothes for days and has a bad odor. She states that George is occasionally very aggressive with other kids and has been in several fights. The teacher also stated that George has been absent on several occasions.
Discuss your approach for conducting a biopsychosocial assessment with George. You are not limited to these questions, but discuss how the following variables could influence your assessment. You will also discuss efforts to ensure your assessment is ethical and considers both the interests of your client (i.e., George) and the public.
