Nurse practitioners are typically working at an advantage over other new providers in that many NP’s are well accustomed to organizing interprofessional collaboration as part of their regular job duties. In fact, as an RN, many nurses are the ones who bring to light issues to the provider in order to suggest a referral or collaboration with other team members such as home health care or a social worker. A patient who may only spend a few minutes each day inpatient speaking with the main physician overseeing their plan may be more concerned with asking when they will be released but fail to mention they are not able to take care of themselves. Nurses that spend more time with patients on average may be able to suggest a referral for occupational therapy or social work to assess the patient’s home needs. This is just one example that shows interprofessional collaboration within healthcare. As APRN’s, we must take this previous experience and integrate into our practice, making each visit patient and family centered. In order to maintain this type of care plan, there must be constant communication between the team members so that each person understands their role (Palanisamy, Taskin & Verville, 2017). It is also important to utilize information systems that are available to coordinate these services appropriately and so that all information gathered can be shared amongst team members for a smooth continuity of care (Abdulaziz, Yasin, AlGamal, Hateem & Ramakrishnan, 2019).
