This assignment is an opportunity to think critically about developing your practice framework when working with clients. In your work with the
client, you will practice from the generalist, strengths, and ecosystems perspectives, person-centered theory, and utilizing the process of case
management.
You will use the action plan that you created for your client as the basis for this assignment. The goal is to determine what practice model/change-
oriented strategy and techniques you will utilize to help your client most effectively reach their goals, how you will ensure that your work
is trauma-informed and culturally responsive, and how you will evaluate progress towards the goal. Think about where the client is in terms
of their readiness for change and what intervention would be most effective in supporting their change process.
Select one of the identified goals from your action plan and build your intervention and evaluation plan around the work you will do with
your client to help them meet that goal.
Content requirements:
? Introduction: Discuss your client’s presenting problem, strengths, and the goal that you have chosen to focus on for this intervention and
evaluation plan.
? Select either the Task-Centered Model or the Crisis Intervention Model to guide your work with your client. Discuss why the model is
most appropriate for a social worker who is working with your client. You will use the information provided in the textbook to discuss why
this model is appropriate (and would be helpful) for use with your client. If you believe that using more than one of these models in
conjunction would be helpful to your client, then please discuss them all.
? Describe (briefly) the procedures/phases/application of the model(s). How will these be helpful in guiding the work with your client?
? Discuss what changes/tasks are on the client’s action plan and how you will follow the procedures/phases/application of the

model when working with your client on the changes/tasks identified. Discuss what intervention techniques you would use to
support your client in completing these tasks.
Be specific about the tools, techniques, and/or activities you would use with your client. You can use resources like the textbook, class
slides, and/or TherapistAid.com for ideas regarding tools and activities.
Examples: Using role play or guided practice to practice a job interview with a client to help them understand and practice what
that encounter will be like with the hopes of helping them decrease their anxiety about the encounter.
? Discuss how you will ensure that your intervention plan is trauma-informed AND responsive to your client’s intersecting identities
(age, race, ethnicity, gender identity, ability, etc.). What will you need to consider regarding your client’s identities as you plan and
move through this intervention? How will you practice cultural humility and anti-oppressive social work as you move through this
intervention?
? Discuss how you will evaluate if the intervention plan is effectively helping your client meet their goals as identified in the action plan.
The evaluation plan should include specific outcomes (increase in positive behaviors/feelings, completion of tasks, etc.) and ways
that you will utilize evaluation tools mentioned in the textbook (pp. 253-260, pp. 434-436). The tools should be as specific as possible.
Example: Using a frequency count if one of your client’s objectives is to increase positive self-talk and/or using the Beck Anxiety
Inventory as a pre-and-post-test to measure the client’s symptoms of anxiety.
You must also discuss how frequently you will collect evaluative information (weekly, monthly, every three months, etc.). The
evaluation should not be based solely on client self-report or social worker observation.
