In this assignment, you will complete a windshield survey for a selected zip code as part of your clinical rotation for the week. (You will select a zip code in consultation with your clinical instructor based on your contributions to Discussion 2.1.)

Download the Windshield Survey Tool from the link below and use it to conduct your survey, filling out each category with notes and observations as you explore the community. The notes that you jot down while conducting the survey will be relatively brief and need further explanation in the next step.

After the survey is finished, read back through your notes and elaborate on them to provide reflections, pose follow-up questions, and establish a basis for further investigation. This is a critical step, as the windshield survey will be the basis for your Community Needs Assessment in Assignment 12.2.

Organize your observations, including your critical reflections and follow-up questions, into a slide deck using PowerPoint, Prezi, Google Slides, or any comparable tool that you are comfortable using.
