Incident reporting is important in healthcare settings because the incident data helps to determine factors contributing to the occurrence of the incident. This information enables healthcare providers to develop and implement interventions to prevent similar incidents from occurring in the future. Reporting incidents also helps organizations improve their risk management programs (Burlison et al., 2020). The data obtained from incident reports is used to measure the effectiveness of risk management programs and identify areas that should be improved. Data from incident reports also enables providers to learn from their mistakes (Burlison et al., 2020). The common incidents that occur in hospital settings are adverse events, near misses, security breaches, and hazardous conditions. Near misses are events that can cause harm but do not result in severe consequences. Security breaches occur when unauthorized people access or use private information. Adverse events are incidents that cause harm, injuries, or death to patients. Hazardous conditions are risk factors that can harm people, property, or the environment.
Incident reports are also used to improve patient safety practices in healthcare settings. Healthcare providers use information from incident reports to develop effective interventions to improve patient safety (Amaniyan et al., 2020). Incident reports also enable providers to identify areas in healthcare that need to be improved to enhance patient safety. In addition, organizations can use incident reports to develop training programs for employees to equip them with knowledge and skills for providing safe care. Hospitals are not required to include data from incident reports in patients’ medical records. Patients’ medical records are strictly meant for patients’ medical information, such as their medical history, diagnosis, and laboratory results. Therefore, there is no need to include incident data in patients’ medical records.
COMMENT 2:
The purpose of reporting and collecting data from incident reports within the healthcare setting is multifaceted and serves several crucial objectives. Firstly, it aids in identifying the possible or root causes of incidents, thereby facilitating a systematic analysis of adverse events (Oweidat et al., 2023). This enables healthcare institutions to implement corrective measures and preventive strategies, ultimately enhancing patient safety and the quality of care (Oweidat et al., 2023). Secondly, the documentation of actions taken in response to incidents is integral for accountability and transparency. It ensures that healthcare professionals and institutions are held responsible for their actions which is crucial for maintaining patient trust and the integrity of the healthcare system (Oweidat et al., 2023).
Falls and medical errors are two examples of common types of incidents found in hospitals. Falls may result from patient mobility issues or environmental hazards (LeLaurin & Shorr, 2019), while medical errors can encompass medication mistakes, misdiagnoses, or surgical complications, among others (Rodziewicz et al., 2023). Reporting such incidents help identify patterns and trends, enabling the development of preventive protocols and staff training programs, thereby reducing the likelihood of recurrence of similar incidents (Rodziewicz et al., 2023).
Hospitals mostly don’t include incident reports in patients’ medical records because details from these reports are usually not directly incorporated into the official medical records to maintain data integrity and compliance with privacy regulations.

The purpose of reporting and collecting data from incident reports in healthcare is to better the patient’s safety by identifying, analyzing, and avoiding potential risks and errors. Incident reports can act as a tool for healthcare organizations to gather information about adverse events, near misses, and unsafe conditions this provides an overview of operational problems. This data enables healthcare providers to understand patterns and causes of incidents which facilitates evidence-based decision-making and the implementation of corrective actions (Sergi & Davis, 2023). The analysis of incident reports supports the development of safety protocols, staff education/training, and quality improvement actions. It is difficult to think that for every one error reported that one- hundred are not (Goda, 2024).
Common types of incidents in hospitals include medication errors, patient falls, infections procedural complications, and diagnostic errors. Medication errors can occur during any use of medication from prescribing to administration. This usually involves incorrect dosages or the wrong medications. This incident came to light to social media when a nurse at Vanderbilt University Medical Center gave the wrong medication that ended up killing her patient (Lusk et al., 2022). Patient falls are also a big problem especially among elderly or mobility-impaired patients, leading to significant injury and extended hospital stays. Infections such as surgical site infections or catheter-associated urinary tract infections have become a frequent issue mostly likely due to limited infection control practices. Procedural complications may involve errors during surgeries or invasive procedures that can cause harm through human mistakes or unexpected outcomes (Gens-Barberà et al., 2021). Lastly, diagnostic errors, such as delayed or incorrect diagnoses, can critically impact patient treatment and prognosis and can be hard to correct.
COMMENT 4:
Under the direction of the director of Risk Management and Safety,the administrator oganizes, monitors, and coordinates the general oversight and processing of District-wide workers compensation and liability claims; acts as a liason with outside administrator,physicians,legal counsel,and employees; performs a variety of administration tasks pertaining to the workers compensation and Liability Claims Program and assist in the investigation of Liability and Workers Compensation Claims.Whiles non-managements staff member role in compensation incident in typical health care organization is to ensure the injured employee receives appropriate medical attention and secure the area to prevent further incidents. They also play the role of reporting the incident to their immediate supervisor or designated safety officer promptly and complete any required incident report forms accurately and thoroughly.
The administrator is responsible for processing industrial injury reports,issues first report of injury to third party administrators,schedules appointment with medical providers when needed. They also coordinates evidence of injuries and illnesses,maintains file and tracks cases in accordance with district policies and procedures.It the responsibilty of the non-management staff member to provide accurate information during the investigation and cooperate with administrators and investigators as needed.
I will recommend that the organization conduct regular training sessions on workplace safety, proper incident reporting procedures, and workers’ compensation rights and also include scenario-based training to help staff understand their roles and responsibilities during an incident.
COMMENT 5:
The administrators and non-management staff roles in in managing workers’ compensation incidents to ensure compliance with regulatory requirements and to maintain a safe work environment are crucial (Bernacki, et al., 2021). The administrator’s primary responsibility is to proactively establish a framework for workplace safety, which includes ensuring that employees are adequately skilled to immediately report workplace injuries and illnesses to Workers’ Compensation (UCDavis, 2019). Their role involves creating and enforcing policies and procedures, conducting regular safety audits, and overseeing the workers’ compensation process to guarantee proper claims handling (UCDavis, 2019).
On the other hand, non-management staff members’ responsibilities include understanding and adhering to established safety protocols, immediately notifying their supervisors of any injuries or illnesses, and providing accurate and complete information for incident reports (Bernacki, et al., 2021). This information is then relayed to the health administrator for further action (Bernacki, et al., 2021).
To enhance organizational compliance at the staff level, I have several recommendations. First, regular and comprehensive safety training should be provided to all employees, emphasizing the importance of prompt incident reporting (Bernacki, et al., 2021). Additionally, incident reporting processes should be simplified and accessible, and supervisors should be trained to handle reports effectively. Periodic audits and safety drills can further reinforce the importance of compliance (Bernacki, et al., 2021).
COMMENT 6:
In a typical healthcare organization, administrators and non-management staff members have distinct roles when dealing with a workers’ compensation incident. Administrators are responsible for overseeing the entire workers’ compensation process, including ensuring compliance with relevant laws and regulations, managing claims, and coordinating with insurance carriers and legal teams to handle disputes or appeals. They also developed and implement policies to prevent workplace injuries and manage risk (Kosny et al., 2011). Conversely, non-management staff members are involved in the initial reporting of the incident and providing accurate information about the injury’s circumstances. They must complete the necessary documentation, cooperate with investigations, and follow the protocols. These roles are crucial for ensuring timely and effective management of workers’ compensation claims, minimizing disruptions, and creating a safe working environment.
In handling worker’s compensation incidents, the minimal responsibilities and reporting duties differ between administrators and non-management staff in healthcare. Administrators must make sure that incidents are promptly reported to the appropriate compensation insurance carrier, maintain accurate records of all claims, and coordinate follow-up actions such as medical treatment and return-to-work programs. They are also responsible for maintaining compliance with state and federal regulations and implementing safety policies to reduce future incidents. Non-management staff members are required to report any work-related injuries or illnesses to their supervisor, provide the details of the incident, and complete an incident reports (Goda, 2024). They must also cooperate with any investigations or follow-up procedures and adhere to noted safety guidelines.
To enhance organizational compliance with regulatory requirements at the staff level several recommendations can be implemented. First, continuous training and education on updates should be provided to all staff with the emphasize on the importance of compliance in daily operations and patient safety. This can be achieved through frequent education. Second, establishing policies and procedures that outline compliance expectations can help staff understand their responsibilities. These should be added into the organization’s standard operating procedures and easily available though whatever education methods they use. Also to create a culture of accountability by encouraging open communication about compliance issues and implementing a reporting system for violations or concerns can help staff feel supported in adhering to regulations (Kwon & Johnson, 2012). Using technology such as compliance management software can make the reporting processes easier.
