Assessing the culture of the Labor and Delivery unit and organizational policies is crucial when planning to implement a nursing practice intervention like hemorrhage prevention in OB patients. The culture of a unit heavily influences how interventions are perceived and implemented. On this unit the culture is one of positive attitudes and welcoming of new people. Considering the culture for change, there is a mix of change proponents as well as employees that are more resistant to change. With a culture that is resistant to change or lacks collaboration, implementing a new practice could have some challenges. The overall culture is welcoming of change, which provides more hopeful outlook for an implementation process. The unit is openly going through several changes already, so I could see this being a barrier in the sense that one more thing for the nurses and staff to attend could add the amount of stress that they feel in the work place. While that could be, I can also see it being a way of stress mitigation in the sense that working on skills and drills could be helpful for the nurses confidence level in their job. This labor and delivery unit has several resources for the nurses including adequate staffing levels, proper equipment to perform their jobs well, opportunities for learning, and multidisciplinary approaches to labor and delivery care. Without adequate support from leadership or access to training, nurses may struggle to adopt the intervention effectively. This unit a few years ago went through safety language training, which has increased the communication satisfaction among staff members. Encouraging a supportive safety culture within nursing units and throughout hospitals leads to a decrease in documented negative patient incidents (Alanazi et al., 2022). Nurse managers play a pivotal role in enhancing nurses’ safety mindsets by advocating for a non-punitive approach to reporting errors and fostering strong teamwork and clear communication among staff members (Alanazi et al., 2022).
I think that understanding the broader healthcare system and clinical system at the local level is vital. This includes knowing how various departments interact, referral processes, and the availability of specialized services. For example, if there are delays in transferring patients to higher levels of care in case of complications, it could affect the timing and efficacy of hemorrhage prevention interventions, or the access to certain specialties that could affect the management of hemorrhages. Effective implementation of nursing interventions often requires collaboration with other healthcare professionals. Understanding how different disciplines interact and communicate within the organization is essential. For instance, if there are communication barriers between nurses and physicians, it could impact the coordination of care and the timely implementation of interventions (Wieke Noviyanti et al., 2021).
Lastly, having mechanisms in place to evaluate the effectiveness of the intervention and gather feedback from staff is critical. Continuous monitoring allows for adjustments to be made as needed and ensures that the intervention remains aligned with best practices. Understanding the culture, policies, resources, patient population, healthcare system, and communication dynamics at the local level is essential for successful implementation of evidence-based practice in nursing (Wieke Noviyanti, 2021). It helps identify potential challenges and allows for the development of tailored strategies to overcome them, ultimately improving patient outcomes (Wieke Noviyanti et al., 2021).
