Support your plan of care with the current peer-reviewed research guideline. The 56 year old African-American Male is diagnosed with hyperlipidemia. This condition occurs as a result of higher blood lipid levels (Stewart et al., 2020). Lifestyle education, diet counseling, and atorvastatin were prescribed for the patient. During this week, I had a variety of cases such as chronic conditions like diabetes and hypertension. Also, we experienced an episode of a cardiopulmonary resuscitation, which was a success. I have worked with multidisciplinary teams and my tasks were mainly on treatment plans, patient education and medication management to achieve the best patient outcomes. The challenge faced was that the patient showed the characteristic of non-compliance with dietetic recommendations. The patient needed assistance to avoid high-cholesterol diet, which became a challenge in the managing of hyperlipidemia. The patient had no insurance and complained about the high cost of medication. Changing the treatment regime seemed more problematic. Over the course of my clinical placement, I was able to teach patients about the pivotal role nutrition plays in their health and provide guidance on how to make healthier choices. Besides, I managed to collaborate with healthcare providers facilitating a more comprehensive approach and tackling some of the obstacles that could arise on the execution of the treatment plans. I experienced the importance of communication and team work. During assessment, the nurse should ask the patient about the timeline of the illness. Here, the nurse asks the patient when he started experiencing the signs and symptoms and evaluate any risk factors such as family history. Perform the physical examination, like that of vital signs and basic metabolic profile to know if the patient is obese. Verify the lab results to be certain that lipids are high in the range of HDL cholesterol and triglycerides. The patient will present with the following symptoms, including yellowish deposits of subcutaneous fat, white/gray halo-shaped corneal area, chest congestion, yellow patches, or nodules of the skin, elevated levels of lipids, and more than average blood pressure (130/80). The patient needs guidance on healthy eating habits. He needs to consume vegetable and fruits regularly. Reduce salt and saturated fat consumption. Promote routine physical exercise as a means of preventing and controlling hypertension. Exercising regularly will help combat high cholesterol. The forms of exercise that will help the patient lower their cholesterol levels include cycling, walking, running and swimming. Exercise is crucial as it will help improve the patient’s lipid profile. Still, the patient should engage in aerobic exercise as they improve lipid profile. The patient should perform aerobic exercise at least 120 minutes per week. I have realized that counseling the patient on the necessity of all-rounding and tailored care plan should be put into focus. I realized that a patient’s education about their medication, treatment options as well as self-management strategies is important. Besides, collaborative care was also a key aspect and further enhanced the patient’s chances of wellness. Most guidelines are being applied to maintain the efficiency of the caregiving plans.
