In 500-750 words (not including the title page and reference page), develop an evaluation plan to be included in your final evidence-based practice project. Provide the following criteria in the evaluation, making sure it is comprehensive and concise:

1. Describe the rationale for the methods used in collecting the outcome data.
2. Describe the ways in which the outcome measures evaluate the extent to which the project objectives are achieved.
3. Describe how the outcomes will be measured and evaluated based on the evidence. Address validity, reliability, and applicability.
4. Describe strategies to take if outcomes do not provide positive results.
5. Describe implications for practice and future research.

Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.

Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.

Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

Introduction

The purpose of this project is to evaluate a process for understanding the impact of multiple interventions on a single student. The process is based on evidence-based practice, which means that it will use research and data-driven decision making to determine which approaches work best for each student. The goal of this project is to make sure that all students have access to high-quality educational opportunities that lead them toward careers in science, technology, engineering and math (STEM).

Executive Summary

The Executive Summary is a summary of the entire project. It should be one page long and should include the problem statement, purpose statement, research questions and significance of solution.

Problem Statement

The problem statement defines the main issue to be addressed. It describes what is not working well and why, who is impacted by it, how it’s impacting the organization, and what are its consequences if left unresolved.

In your proposal, address these questions:

• What is wrong with our process?

• Who is being harmed by this problem?

• How are we hurting ourselves or others by staying in this situation?

• What needs to change in order for us to move forward successfully?

Purpose Statement

The purpose of this evaluation is to determine if the process is effective or not. This will help us improve our work and make better decisions in the future.

The evaluation will also help us see if we need to change our ways, as well as how they could be changed if they’re not working well enough for us!

Research Questions

The research questions for this project are:

• What is the problem?

• What is the solution?

• How will you know if your solution is working?

• How will you know if your solution is successful?

Significance of Solution

Your proposal will be evaluated on the basis of its significance to the organization. To be considered significant, your solution must have a negative impact on the organization and its stakeholders. It must also be widespread in the industry—that is, it has been identified as a priority by leadership at your organization or another organization that’s similar to yours (like an association or trade group).

Project Goal

The goal of the project is to evaluate the process used by the school district to implement evidence-based practices in their schools. The purpose of this evaluation is to determine whether or not an improvement in student achievement has occurred as a result of implementing these practices. This will be done by comparing students’ performance on standardized tests before and after implementation, as well as looking at other factors such as attendance rates and graduation rates.

In order for this evaluation to be valid, it must take into account:

Conclusion

The purpose of this research paper was to evaluate the current process for when a patient is referred to a specialist. The results show that there is room for improvement, and that most practices do not follow proper procedures when referring patients.
