Based on this information we decided to develop an application that integrates disease management and access to a healthcare provider. HTNTrack App is a phone application, developed by pharmacists, that mimics a hypertensive journal with the uniqueness of having a pharmacist On-Call 24/7. “HTNTrack APP” is a tool that will bring patients, caregivers, and healthcare providers a space to keep track of the patient’s hypertension. The customers will consist of hypertension patients, caregivers, and healthcare providers.
