As an example I may choose the topic of depression, as I am interested in all of the depression I am seeing in the primary care office. Next week I will explore the background of this topic and this will lead to my PICOT question.

Post your background question and your strategy for getting a comprehensive understanding of the clinical issue. Your background question will lead you to your foreground or PICOT question–so it is an important piece of the project!
Your background question should answer the who, what, when, where, how and why. With a clinical question you will include the definition, epidemiology, clinical presentation, diagnosis and conclusion. As an example, when I am thinking of background questions that I would like to answer for my topic of depression I would ask:
1. How is depression defined and how is it diagnosed in primary care?
2. What population is at the most risk for depression?
3. What are some of the causes of depression?
4. How is depression identified and treated?

We will be reviewing a “keeper study” this week—evaluate an article by working through the following 7 question
Again, keeper studies can be identified using handy Rapid Critical Appraisal checklists consisting of a set of simple but important questions. Below are sample questions developed for use with quantitative studies that are applicable to most appraisal situations (it’s important to note that qualitative evidence, if it’s relevant to the clinical question, should not be dismissed):
Why was the study done? Make sure the study is directly relevant to the clinical question.
What is the sample size? Size can and should vary according to the nature of the study. Since determining a valid minimum sample size in a single study can be difficult, taking into account multiple studies is beneficial.
The answer to this question alone should not remove a study from the appraisal process.
Are instruments of the variables in the study clearly defined and reliable? Make sure the variables were consistently applied throughout the study and that they measured what the researchers said they were going to measure.
How was the data analyzed?
Make sure that any statistics are relevant to the clinical question.
Were there any unusual events during the study? If the sample size changed, for example, determine whether that has ramifications if you wish to replicate the study.
How do the results fit in with previous research in this area? Make sure the study builds on other studies of a similar nature.
What are the implications of the research for clinical practice? Ask whether the study addresses a relevant and important clinical issue.

To understand synthesis it is important that you read the lectures for this week. Synthesis is a skill that takes practice and the lectures discuss this process.
On the discussion board, using a minimum of two articles that are supporting your PICOT question, submit one paragraph synthesizing the research into clear, concise statements without separately reviewing each of the studies in the paragraph—but by paraphrasing and synthesizing the work that was done.

This week you will be creating an Implementation Plan that will be attached to your final Evidence Based Project. In the article read last week, “EBP Step by Step Following the evidence: Planning for sustainable change” an Implementation Plan is provided for an example. In your text (4th ed) the ‘ARCC Model Timeline for an EBP Implementation Project” (Appendix I) template is available for guidance.
The thought process includes: Based on my review of literatures comparing the DASH diet and the low/restricted sodium diet, the DASH diet significantly reduced the blood pressure of the hypertensive patients in the studies. The effect was additive when both diets where combined.
These studies established evidence that the DASH diet in addition to pharmacotherapy is effective in reducing the blood pressure of hypertensive patients.
My objective now is to introduce this diet method (DASH diet) to all the hypertensive patients that will be seen in my place of practice. My implementation plan will involve the identification of the other stakeholders/beneficiaries of this new method or approach to management of hypertensive patients. The other stakeholders mentioned in my implementation plan were the healthcare providers (ARNPs and physicians), the insurance companies, and the owner or employer of these healthcare providers.
I intend to implement my EBP through discussion groups. These identified stakeholders will be invited to a group discussion so that barriers will be identified, and solutions will be worked out during these group discussions.
For the discussion board this week—outline your implementation plan–the early steps have been complete–the checkpoints noted in the template should guide your plan.
You have found support for your question–now you are translating this evidence into your practice.

We used a PICOT question to assist us in our search for evidence–thus making you think there is research involved in your project. In truth, I can understand the confusion–but the purpose of the PICOT is to assist in your search for support in implementing the change. Once you have the support then you are evaluating the change not the question.

This week you will be submitting your EBP Project to the assignment link and start working on your presentation. You will be submitting a draft of your presentation to the discussion board this week and the final presentation to the discussion board in week 10. In week 9 you will gather the feedback from your peers so as to present the best presentation you can in your final presentation.
Post your Draft Presentation of your EBP on Discussion Board for feedback before final submission of your presentation will be in Week 10–the presentation can be a power point or any of the presentations methods discussed this week in the lectures and in your reading.
Please note that the assignment this week is submission of your final EBP project.
