Clerk to:

prepare Authorization & Direction for RS to sign for release of all medical notes from their doctor, Dr. Natalie  Vander (NV) – clerk to confirm address of Dr. NV

begin drafting statement of claim – see notes in file from previous meeting on September 21,2022 with RS

clerk to draft letter to client – arrange meeting in next 2 weeks and suggest 3 possible dates; provide contact info for psychologist and include consent to be signed by RS to meet with neurologist, Dr. Sara Monner, at Sunnybrook Hospital in Toronto.
