Before you begin your clinicals, you must upload your FULLY COMPLETED student preceptor contracts to this assignment. Upload ONE completed and signed student preceptor contract(s) (word) per preceptor.

Your clinical hours will be void until all fully completed document(s) are submitted.

The best way to submit your file is to scan the downloaded and signed contact; however, if you do not have a scanner, it is permissible to use a smart phone. You may use an app such as Tiny Scanner to scan your document for upload, or you may take a photo of the document and email the signed document to your Regis email account. You then can download and save the document on your computer. When completing the assignment, you then can upload the document into the assignment section for submission. The document must have a handwritten signature from your preceptor and all areas of the form completed to be accepted as “PASS” for the assignment. Students MAY NOT start clinical until this assignment has been completed and passed.

This assignment is due no later than day 7; however, the sooner the assignment is completed, the sooner clinicals can begin.
