Description
Assignment Directions:
This project will require you to complete several components of which the culmination will be the development of a final paper. Each of these components is designed to contribute to the final paper. You will have the opportunity to identify and interview a caregiver. You will choose someone who is experiencing a role transition while providing care of a family member recently diagnosed with a chronic condition. The chronic condition must be new or recently diagnosed (less than one year), thus the occurrence of role transition will also be new. Through the interview process, you will apply your knowledge by providing education and resources to the caregiver/advocate and family member.
This paper will be a compilation of the information learned over the previous weeks. The application of knowledge acquired will be applied as you conduct the interview, educate and provide resources and synthesize the information in the paper.

Paper Format:
1. 4-6-page APA formatted paper. Limited page numbers encourage the student to be organized and concise in their writings. The total number of pages excludes the title and reference pages.
2. The paper formatting must include a correctly formatted title page, headings, an introductory paragraph, the body of the paper, a conclusion paragraph, and a reference page. An abstract is not required.
Introductory paragraph- State clearly the purpose of the paper. What is the main reason for writing the paper? Explain briefly the major points to be covered in the paper and why readers should be interested in the topic.
Body of paper- Present the required information. Every point should be supported with examples. Cite the information and include the cited references on the reference list. Use APA formatted headings that reflect the six (6) content areas listed above.
Conclusion paragraph – Summarize the key points in the body of the paper. Do not introduce new information in the conclusion.
Headings- Follow correct APA formatting for all headings.
3. The paper should have a minimum of five (5) peer-reviewed journal articles. You are welcome to use informational websites but they will NOT count towards the minimum five peer-reviewed journal articles. All references must be cited within the paper. Required readings from the units may be used but do NOT count towards the minimum five peer-reviewed journal articles.
4. The paper should be written in third person, not first person. The information for section 6 listed above, can be written in first person. Although the paper will be based on an interview, do not write the paper in a conversational format.
5. Do not cut and paste the information from the literature review into this paper. It disrupts the flow of the paper and points have already been received for this section of the assignment. It is expected that pertinent information from the literature review will be selected and correlate with the information in the paper.
6. Use scholarly terminology in the paper. Avoid use of colloquial expressions and slang. Because some students may choose to interview a family member who is a caregiver, the paper may tend to take a more casual approach. Please maintain professionalism and scholarly terminology at all times.
