PICOT: For adults in a long-term care facility, implements the AHRQ Fall Prevention Program, compared to current practice, impact fall rates in 8 weeks?
Part I: All Students
Please answer the following questions:
1. As you complete your project implementation (or case study simulation), please discuss how your practice change intervention will improve nursing practice and patient outcomes.
2. Discuss your thoughts on your own project outcome and if other variables influenced your outcome.
Part II: For Case Study Students
Upon completion of the simulated NR-707 Case Study: In Week 8, please upload the completed Case Study template as a Word document, containing all 8 weeks of your case study discussion responses, as an attachment to your first response in the Week 8 discussion. The template is due by Wednesday 11:59 p.m. MT of Week 8. All 8 weeks of the Case Study should be included in the Word document.

· Discuss best practices for implementing evidence-based practice guidelines with references sufficient to support best practice.
· Discuss strategies for overcoming barriers in implementing evidence-based practice with references sufficient to support strategies.
· Discuss your formative evaluations. How are you addressing challenges and providing feedback?
· Describe your sustainability plan:
o Discuss how the stakeholders will maintain intervention fidelity moving forward.
o Are champions appointed to assure sustainability?
o Does the site plan to implement this project on other units?
o Are there required changes to the technology to sustain your project? If so, how will your champions advocate for these changes?
o Describe how your project will become the new standard of care and include how the data may be used to inform health care policy for this unit.
o What changes do you believe would be needed at the micro, meso, and macro systems levels to ensure this project’s continuation?
· How will you plan to disseminate your project outcomes?
