CC: “I’m just here to do a checkup. I’m starting a new job, and the company requires me to do a physical checkup.”
HPI: A.G. went to the clinic for a required physical checkup for his new job. The last physical was a year ago, in 2020. He complained about a headache 3-4x a month. The symptoms were not so severe as to cause him to seek treatment. The patient would take over-the-counter Tylenol 500mg or Advil 200mg once or twice a day, depending on the headache. No other complaints were verbalized.
PMH
Medical Problem list: The patient stated that he is anemic
Surgeries and hospitalizations: Deny prior surgery and hospitalizations.
Immunizations: The patient received the flu vaccine last November or December 2020 and received his first dose of the Covid 19 Pfizer vaccine last June 2021.
Allergies: The patient has seasonal allergies, especially around Springtime. He takes Cetirizine10mg once or twice a day. Denies allergies to food and drugs.
Medications: The patient has no prescribed medications but would take Tylenol 500mg or Advil 200mg for headache and Cetirizine 10mg for seasonal allergies. The patient sometimes takes multivitamins and verbalized taking Collagen Powder for his skin.
Chemicals: Patient drinks 3-4 servings of alcohol 2x a week for pleasure. The patient smokes cigarette 3x a month with friends to socialize. He drinks 2 cups of coffee in the morning and a cup of tea at night. Patient denies use of illicit drugs.
Social and Family History:
Occupation, marital/relationship status & current living situation: A.G. is a pharmacist, and his highest level of education is a Doctorate Degree. Patient is not married and has no kids. He is currently living with his parents and 2 siblings and feels safe at home.
Sexual/Reproductive History: The patient’s first sexual encounter was when he was 17. Patient is currently seeing a female partner and is not using a protection. Patient stated that he never had any STIs from the past and was last tested 6 years ago. According to the patient, his partner gets tested.
Spiritual/Social Supports: The patient feels comfortable to share problems with friends and family.
Diet/exercise: According to the patient, he does not have any specific diet that he practices. He mostly eats at home and would sometimes eat fast food 5x a week. A.G. exercises every day for 30-40 mins a day by playing basketball or riding his bike.
Safety: Patient wears seatbelts while driving. He sometimes texts while he drives, but denies driving when drunk. Patient denies owning a gun and has never been involved with domestic violence.
Family History: MGM has arthritis. She died at age 75 about 10 years ago. A.G. is not sure if MGM died due to CKD. MGF was diagnosed with Alzheimer’s 3-4 months ago at the age of 84/85 and is still alive. FGM died a month ago from old age of 90 years old. FGF died at age 67 due to Leukemia. Mom is anemic and has no prescriptions. Dad has high blood pressure and is taking Losartan 10 mg. Brother is anemic and overweight, but no prescriptions. Sister is also anemic and had childhood asthma, she was taking an inhaler for it before.
Review of Systems: The patient has been feeling tired lately due to working out more. He also stated that his anemia causes it too. He denies any changes in sleeping patterns and is not experiencing any fever, chills, or night sweats. Patient denies feeling of depression, anxiety, and suicidal thoughts. He complains about a headache 3-4x a month and takes Tylenol or Advil for it. He denies having difficulty of breathing, wheezing, chest tightness, pain while breathing and coughing. Patient also denies N/V, stomach pain, change in bowel, heartburn, constipation, diarrhea, flatulence, and bloating. He does not experience any painful urination, does not wake up at night to urinate, denies blood in urine, flank pain, UTI, discharge, and can hold urine if need to. He denies any muscle pain, joint pain, muscle weakness, and joint swelling. He does experience back pain every time he works with cars. Patient had 2 fractures growing up. The first one was when he was 14 years old, he fractured his left ankle from a scooter accident, and the other one was when he was 22 years old, he fractured his left pinky toe from playing soccer. Both fractured did not need any surgery and was fixed with a cast. The patient never experienced passing out, dizziness, vertigo, light headedness, spots in eyes, no tingling sensations, did not noticed being clumsier, loss of sensation, seizure, loss of balance, and memory loss. Patient has no bleeding, bruising or frequent illness. No unusual weight loss or weight gain and can tolerate heat and cold well.
Risk Assessment:
1. Risk of CVA (stroke) due to smoking, age, obesity, and family history (CDC, 2021)
2. Risk of unintentional/accidental death, due to distracted driving (CDC, 2021).
3. Risk of T2DM; due to family history and diet (Hollier, 2021).
