Reflective Journal, Simulation Debrief, and Report Submissions
For this assessment, make sure that you submit:
The PDF containing all required entries of your reflection journal. When you have completed all four entries for the Nursing Reflection Journal: Four Spheres of Care activity, click Output Activity to download a PDF of your answers. Then submit the PDF to this assessment.
The completion reports for the following Sentinel U simulations (report must show completion percentage, points, and time spent in simulation).
Family Support and Home Assessment v5.
Prioritization of Care Geriatrics v2.
Prioritization of Car Adult Medical v2.
Patient Management & Delegation v2.
The four (4) EMPOWER debrief reports for the following Sentinel U simulations Each section on the Empower Debrief tool must contain at least 100 words.

Be sure to review the Grading Guide criteria and that you understand how your grade will be calculated using the Sentinel U reports. A total of nine files must be uploaded.

Proficient
Achieves a minimum of 75%-84% on one or more of three sections, Family Support Assessment, Apartment Assessment, and Nursing Diagnosis AND submits a complete EMPOWER debrief report for Family Support and Home Assessment.
Basic
Achieves a minimum of 60%-74% on one or more of the three sections, Family Support Assessment, Apartment Assessment, and Nursing Diagnosis AND submits a complete EMPOWER debrief report for Family Support and Home Assessment.
Non Performance
Achieves less than 60% on any of the three sections, Family Support Assessment, Apartment Assessment, and Nursing Diagnosis OR does not submit a complete EMPOWER debrief report for Family Support and Home Assessment.
Criterion 2
Upload the report for Prioritization of Care Geriatrics Sentinel U Simulation and associated EMPOWER debrief report.
Distinguished
Completes 100% of simulation AND receives a score of 85 points or greater AND submits EMPOWER debrief report for Prioritization of Care Geriatrics.
Proficient
Completes 100% of simulation AND receives a score of 75-84 points AND submits EMPOWER debrief report for Prioritization of Care Geriatrics.
Basic
Completes 100% of simulation AND receives a score of 70-74 points AND submits EMPOWER debrief report for Prioritization of Care Geriatrics.
Non Performance
Does not complete 100% of the simulation OR receives a score of less than 70 points OR does not submit EMPOWER debrief report for Prioritization of Care Geriatrics.
Criterion 3
