For Task 3, you will apply these essential skills in a real-world scenario using the simulation platform, Mursion. During this simulation experience, you will interact with a client (avatar voiced by an actor) with the goal of agreeing on a care plan for the client. To prepare for this simulation, read the scenario description and attachments for information about the situation and the characters who will be part of your simulation experience. You will use this information to create a plan that will help you during the simulation to focus on the main objectives of Task 3. You will submit your task, including the care plan and reflection, but you do not submit the simulation recording. The recording is for your reference since you must use specific examples from the simulation in your plan and reflection.

Scenario
You are a community health worker working at the Center for Coordinated Homeless Services. Your client is a 45-year-old man named Don, who is currently unhoused and has inquired about receiving assistance. He has a history of severe depression and Type 2 diabetes, but he lacks consistent medical care, and he cannot afford his prescribed oral medication. Don does not have a phone or source of transportation, and he lacks family support. He has just been released from the emergency room (ER), and a social worker referred him to your organization.
Currently, Don needs assistance in finding stable housing and securing employment, specifically in construction since he has prior experience. Additionally, he has expressed that he will not go to nearby homeless shelters since he is a dog owner, and dogs are not allowed on the premises. He is adamant that his dog, Beau, is his best friend and he will not go anywhere without him. Beau is a five-year-old canine male who weighs approximately 15 pounds and is partially trained. When Don was taken to the ER, Beau stayed with a friend of Don’s who is also unhoused.
You are tasked with developing a care plan for Don that addresses his most significant problems or needs using the information in the scenario and the “Task 3: Clinical Notes document.” You will present this care plan to Don in the Mursion simulated meeting and make revisions as needed after your discussion.
Prepare for your meeting with Don in the Mursion simulation by doing the following:
Plan to use the OARS interviewing technique to collaboratively develop a care plan with Don to address his most significant needs.
1. Develop five open-ended questions specific to Don’s case.
2. Identify two opportunities to give Don affirmation using details from the scenario.
3. Prepare two strategies for demonstrating reflective listening.
4. Explain how you might use summarizing when meeting with Don to collaboratively develop Don’s care plan.
Develop a care plan to connect Don with community resources to address his problems or needs. Include the following in your care plan:
1. Describe two healthcare related resources for Don that address his most significant healthcare related problems using specific examples from the scenario or simulation experience.
2. Describe two community resources for Don that address his most significant problems related to food, housing, education, and other social needs using specific examples from the scenario or simulation experience.
3. Identify three community or public health professionals that could be involved in Don’s care plan.
a. Describe one specific contribution that each of the three professionals would offer to the care plan.
4. Describe two potential socioeconomic or physical barriers that may hinder Don’s access to the resources in the care plan.
