Scenario
A 75-year-old male patient, Max, has multiple comorbidities, including congestive heart failure (CHF) and osteoarthritis. Max and his wife travel 50 miles to the clinic; their community has a healthcare professional shortage. Max’s support systems include his wife and two children who live locally.
To assist with the management of his congestive heart failure (CHF), Max sees a cardiologist at the outpatient clinic. Max recently experienced a significant exacerbation that could not be managed with a visit to the clinic, his family decided to take him to the emergency room. He now has been hospitalized for management of his congestive heart failure three times in the past 12 months.
During a hospitalization last year for CHF, Max’s condition required a transition to the use of oxygen at home, which he did not previously require. After a hospital-based physical therapy evaluation, it was also determined that he would need a walker to ambulate because of both the CHF and the osteoarthritis.
Due to living in an area with a healthcare professional shortage and his wife’s lack of a driver’s license, Max has often dealt with transportation access issues, which have caused him to miss appointments. His cardiologist believes that these missed visits contributed to an increase in hospitalizations for CHF exacerbations.
