Review the following case study:
Mr. Henry Navarro, a 62-year-old Hispanic male patient, is coming in to establish care with you today. He states that is feeling well, but he has noticed some tingling in his feet and rashes in his private area. In obtaining his medical history, he reveals that he had a cardiac stent placed about 6 months ago and was given some medications from his previous primary care provider and cardiologist. He ran out of his medications and does not remember what they were. However, he does remember that he was taking a blood thinner and something for high blood sugar, blood pressure, and cholesterol. He also relates that he has started feeling depressed since he does not have energy and has lost interest in his usual activities. He has also noticed some productive coughing and shortness of breath (SOB) after exertion. He states that he has smoked cigarettes since he was 16 years old, and he continues to smoke one pack per day.
After a thorough physical assessment and a review of documents, you have gathered information about his following conditions:
• Type 2 Diabetes: His last A1C was 7.2, and he is on metformin (500mg two tabs BID).
• HLP: He is not on any medication now, and his last LDL was 102, HDL: 38, and Triglyceride: 212
• HTN: He is not on any medication, and his blood pressure is running in 150s /100s.
• Depression: He is not on any medications. He is feeling very depressed and wants medications to help him feel better. He denies any suicidal or homicidal ideation.
• CAD: He was told to take a low dose aspirin once a day, but he is not taking it. He wonders if he should.
• S/P Cardiac Stent Placement x 1: He was last seen by his cardiologist over 6 months ago. He was on a blood thinner, but he does not remember its name.
• COPD: He was not diagnosed until he was hospitalized. He has an albuterol inhaler, but he is still feeling SOB. He asks if you can give him something to help with his cough and SOB.
• Peripheral Neuropathy due to Type 2 Diabetes: He would like medication to help with the tingling sensation and pain in his feet.
Review the previous case studies you submitted in the Week 2 and Week 4 summative assessments and consider the evidence-based guidelines for COPD and depression. Note: You will address COPD and depression as well as all of Mr. Navarro’s conditions in this assessment.
Assessment Deliverable
Create an 8- to 10-slide Microsoft® PowerPoint® case study presentation in which you outline medication treatment options, patient education, and holistic care for all of Mr. Navarro’s conditions. Note: The title and references slides do not count towards the overall slide count.
Address the following parts in your presentation:
Part 1: Medication Treatment Options
• Outline the most appropriate medication regimen for all of Mr. Navarro’s conditions based on current evidence and guideline recommendations (e.g., ADA, AHA, GOLD, APA).
• You may use information from previous assessments in this course on the following conditions:
• Type 2 diabetes
• HTN
• HLP
• CAD (post-stent)
• Justify your selections based on pharmacological principles, guideline recommendations, safety, and patient-specific factors.
• Discuss the potential side effects and drug interactions of each medication. Consider the following:
• Renal, hepatic, and cardiovascular status
• Polypharmacy
• Age-related changes in pharmacokinetics
Part 2: Patient Education and Holistic Care
• Develop a culturally responsive and health-literate education plan that addresses the following:
• Medication adherence
• Smoking cessation
• Diet, exercise, and foot care for diabetics
• Signs of hypoglycemia, worsening SOB, and mood swings
• Recommend preventive strategies (e.g., vaccinations, follow-up labs, depression screening).
• Discuss barriers to care, including language, affordability, and cultural beliefs.
• Include referrals that are to the correct individual and group (e.g., smoking cessation program, behavioral health, cardiology, pulmonary).
