This 53-year-old male patient comes to the emergency department after being found unresponsive. Patient has a known history of lung carcinoma and has been told to expect this outcome at some time in the near future.
Patient was found by paramedics in sinus bradycardia with a pressure. IV started and patient was intubated on the scene. Atropine and epinephrine were administered on several occasions without development of a pressure. Patient placed on pacemaker but no pressure or pulse developed.
On arrival, no pulse, no respirations, no blood pressure, undergoing cardiopulmonary resuscitation. Pupils fixed and dilated; corneal reflexes absent. Heart sounds not heard, no spontaneous respiratory effort. No peripheral pulses, femoral pulses felt. Patient has been unresponsive without blood pressure or pulse for approximately 45–50 minutes and pronounced dead.
