1. Unfreeze: Summarize Lewin’s “unfreeze” phase and explain it in relation to your practice problem and corresponding change plan (2–3 paragraphs).
a. Look over the evidence that supports the change or similar interventions (use at least 3 scholarly sources).
b. Identify the stakeholders who would need to be informed of or involved with the change.
c. Explain who would be impacted by the change and how.
d. Describe the driving and restraining forces behind this change.
2. Change: Summarize Lewin’s “Change” phase and explain it in relation to your practice change (2–4 paragraphs).
a. Provide an overview of the change plan.
b. Outline the steps or phases of the change plan. Include a timeline for various steps/phases (depending on your change plan, the time-to-completion may vary and does not have to be limited to the length of the course).

d. Identify resources needed to implement the change (i.e. funding for staffing).
