An example of a system failure within a company or organization is the U.S. Department of Veterans Affairs. They are famous for extensive wait times and poor record keeping. Often times the VA is using outdated equipment and has underpaid employees thus resulting in unmotivated workers. Lastly the VA really doesn’t have any form of accountability as they are rarely ever audited. The VA needs an entire rebuild from the ground up and needs more sufficient funding to attract better doctors, nurses and administrators. Additionally, the VA needs to partner with more organizations and create a wider network to ensure they are being kept up to date with the latest tech and processes. Lastly, the human care factor needs to be reintroduced into the VA. Unfortunately speaking with someone at the VA can feel like going to the DMV, where employees are just there to work a job and have little care about the wounded or disabled veterans they are suppose to be supporting.
