As a healthcare professional in a local walk-in clinic, you have been asked to meet with an 85-year-old male patient who has recently suffered from frequent falls. After noting several instances in which interprofessional care coordination needs improvement, a care coordinator in an acute care facility in a rural farming community is working to establish a new evidence-based practice (EBP) discharge protocol. The facility has seen an increase in readmissions and in patient dissatisfaction and, as a result, an increase in the length of stay and cost of care. The increasing number of patient transitions between departments and institutions requires effective patient handovers, especially those of frail patients with comorbidities. Continuity of care at patient discharge from the hospital is a critical aspect of high-quality patient care.
Public health/policy
A state public health official has joined a committee of non-profit organizations and is interested in decreasing health inequity, specifically the social determinants of health (SDOH) associated with the increasing mental health concerns in military veterans. The committee’s goal is to identify and prioritize current community determinants to create an action plan, including the consideration of health inequality EBP, action planning EBP, specific demographic gaps in care (population health), community demographics and gaps in care needs, and the best interventions for this community’s cultural/demographic/ health status.

1. Describe how each of the evidence-based practice process steps was used to address the question.
2. Include an assessment of how the client’s location, background, and circumstances will affect outcomes both currently and in the future.
B. Select a resource from the WGU library to inform evidence-based practice (EBP) recommendations to address the question posed for the chosen scenario.
1. Include citations from three WGU resources with an annotated bibliography. The resources must be from the last 5 years.
2. Assess the quality of the research information used to form evidence-based practice (EBP) recommendations to address the needs of the client.
