They have asked for you to develop and present a logic model to address the gap as well as innovative
programs to meet the needs of the population you serve.
You need to assess the current resources that are available within your community, the gap in services, and the use of best practices to identify recommendations for the new program to enhance interventions for clinical practice with families groups, organizations, and communities. You will present this logic model in a video-recorded presentation to the hypothetical grant funders in which you will also discuss and defend your recommended interventions.
Instructions
Building from the topic or population you chose for your Week 6 group assignment or using a different topic of your choice, you will create a logic model that you will also present in a video-recorded presentation that identifies resources and best practices that address your chosen issue or population. Please complete the logic model and share it during your video-recorded presentation. You will submit the presentation and completed

● Provide brief and concise statistics that identify the community problem you
are analyzing. Clearly state the problem statement.
● Assess the gaps in service in the community and provide a synopsis of what
programs are missing to address the issue.
● Provide at least two smart goals that provide insight into the innovative
organization of community- and evidence-based intervention you are
recommending to address this issue.
● Identify a minimum of six inputs needed for the recommended evidence-based
interventions.
● Discuss the evidence-based activities to best impact this community need
through your innovative intervention. (Identify at least three.)
● Discuss the research-based expected output measures as the result of the
activities.
Additionally:
● Each logic model must include a title you give to a community issue and the
community for which the model is being developed.
○ Define the problem and some current statistics facing your
community about your chosen topic.
○ Research and address the gaps in services in your community, what
services are available, and where clear gaps appear in service.
○ Discuss two measurable smart goals that your community could
establish to address this issue.
○ Discuss the inputs an agency or community would need to offer the
intervention.
○ Evaluate evidence-based research on which activity or intervention
you would recommend to address this community issue.
○ Analyzing the research and discuss what literature shares about
expected outputs for this recommended intervention.
○ Discuss your recommendations for the next steps for the community
to start to address your chosen topic.
