The Constant Changes in Patient Care
Reply:
In my clinical setting, many of the theories can play a role. Given that Nursing is an ever-changing environment, it is essential to be flexible with the job. The classical theory relates to my career in that you will have those who are more experienced than others, and it helps me be more of a leader and team player, knowing my roles and my boundaries and having confidence in my strengths, as well as my weaknesses. The chain of command can be well illustrated in the ICU because there will be times when I need more expertise and help in situations I have not encountered before (Masters, 2020, p. 325). Systems Theory plays a part in knowing who and what your resources are in the unit. We also need to understand the importance of costs, products, and what output we might need to produce with patients before discharge. There are many moving puzzle pieces when patients are in the ICU because care can become complex. With contingency theory, the Native American population is vast in Montana; as student RNs, we were required to take a Native American History course to help us better understand the culture of the different tribes. The hospital where I work has a Native American Enrichment center for families and patients. People travel from towns far away where tribes reside, which is helpful in many ways. Chaos theory relates most to me and the environment I work in. Some days, I solely rely on routine with less complex patients, and others are, as I say, “winging it.” There is a sort of adaptability and flexibility concerning chaos theory (Masters, 2020, p. 325). You learn in the ICU because of how rapidly things can change. Lastly, Complexity theory is the amount of communication you can have across the board with providers involved in the patient care and the other team members, such as the many nurses coming through to care for the same patients (Masters, 2020, p. 325). This theory explains how sometimes things can be like a long game of “telephone,” and if we do not communicate and adapt, things might get out of control. This theory is vital because of patients’ long journeys in the ICU.
Nursing leadership is essential and something I want to grow in each day. Nursing is constantly changing with our patient care in their medications, routes to recovery, and frequent studies. These theories map out an organized way to help grow in leadership roles and support the future of nurses. “Nurse leaders make a difference in workplace culture and drive positive changes in health care legislation. When a team admires the qualities of their leader, it boosts morale and promotes a psychologically safe workplace, which leads to higher job satisfaction and retention rates. Influential leaders in nursing ensure that the organization’s vision is communicated to the nursing staff while mentoring the nursing leadership of tomorrow (ANA, 2023).”
