One of the ways that I apply Continuous Quality Improvement into my daily nursing duties is to recognize and track trends. A current trend we are noticing is the increase in Urinary Tract Infections. Some patients are prone to UTI’s and those with catheters are going to have a Urinary Tract Infection more frequently, so we look at the residents who are having first time infections, or those with reoccurring infections. In this instance, we met as an interdisciplinary team and decided that in addition to the increased fluids and antibiotic treatments, we needed to in-service our team on proper peri-care.
To do this, we first identified all the members of the care team that would benefit from the educational opportunity. This included the NA’s, CNA’s, MAC’s, Nurses, and members of the admin team who hold a certificate or license. Next, I checked my schedule to see when I would be able to host a skills check-off in the classroom and settled on the first week of Amy after this current class is complete. We reviewed our policy and compared it to current standards to ensure we are accurate and up to date. I try to make checkoffs less daunting to the team and more of a fun learning experience, so I invited a few community partners to provide door prizes and snacks.
Identifying areas of opportunity in your day-to-day practice can contribute to an overall improvement of care.

Safety and Quality are essential when it comes to healthcare. Continuous Quality improvement is based on the notion that there is always opportunity for improvement. Quality improvement strategies are developed by teams for continuous improvement. I currently work in home healthcare. I am fortunate enough to get paid to take care of my daughter. But if I was in another person’s home the CQI would be different. The quality of care that I would provide for that child or adult would depend on the needs of the patient. In home health care we go into patients’ homes to provide a service for their loved ones. The company that I work for values diversity and inclusion. Aveanna has multiple programs to help create this environment. The facility tries to retain people of all nationalities and cultural backgrounds to provide a diverse workforce. Aveanna strives for quality improvement by having the highest level of training programs, investing in clinical documentation and EHR. Aveanna is very agile and keeps a mind set focused on constant change. This allows for the continuous quality of patient care.
I currently work with Pediatrics only, so we have not only the patient to think about but the parents too. It is essential for the parents to feel comfortable with the people that the agencies place in their homes to care for their children. It is important that those nurses are culturally aware of the families they care for to provide high quality care.

When you work in an acute-care setting, like a hospital, continuous quality improvement is well known to the care team providers. We want the products that we utilize for patient care to improve to the highest standard possible to provide the best possible outcome for our patient. One example I can think of is the reduction of HAPIs, or Hospital-Acquired Pressure Injuries. When a patient is put on bedrest or is unable to independently reposition, their pressure injury risk skyrockets. With the use of CQI, there are now several different interventions used by bedside nursing to prevent HAPIs, just as 2-hour turns, appropriate wound dressings, and the Braden Risk scale. In my hospital organization, we recently started using wedges which have been found to reduce the pressure on high-risk points as opposed to using pillows. We’ve also started using specialty cream for patients at risk for HAPIs, such as patients with incontinence issues or bed rest patients without any active pressure injuries. CQI is constantly changing this area of care and a method that would best support this is the Kaizen method, which is the “philosophy of gradual, incremental and orderly continuous improvement, always working to create more value and less waste” (PHAB, February 2020). This method works to incorporate small changes to identify issues and make rapid improvements which is very important and essential in the healthcare world because we want our products and methods to optimize our patient’s care and outcome.

Continuous quality improvement (CQI) in healthcare is a systematic approach to improving patient safety andk care. This process is essential to nursing practice, as it helps ensure patients receive the best possible care.
Quality improvement in nursing involves identifying and addressing problems in healthcare delivery to improve outcomes. CQI uses data to identify improvement areas, develop and implement interventions, and evaluate the results.Kakacek, B., (2023)
An example of CQI, that I used in the past whilst working in an outpatient dialysis clinic was applying measures to promote safety and prevention of catheter related infections. For a while maybe two to three months, we noticed an increase in catheter related infections in our clinic. Upon careful investigations we discovered that patients were taking showers and getting the catheters wet which overtime led to the rise in infections. So, the Nephrologist began placing fistulas to limit and get rid of the catheters, but fistulas carry a maturity time of approximately 6 weeks. The manager and nurses deviced a plan to optimize care and prevent fatalities, so we gave patients plastic wraps for the catheters to prevent it from getting it wet completely and focused a lot on education in the clinic.
The CARE model emphasizes upfront care and creates a system where more productive interactions take place between patients and healthcare providers during the care process. According to the CARE model, patient care should be safe, effective, timely, efficient, equitable, and patient-centered. There are many nursing organizations out there in the world supporting nurses and giving them a voice. Now some organizations are specific to certain specialties. For example, for my area we have American Psychiatric Nurse Association which provide a wide range of information such as memberships, resources, news, and educational services. These sites give nurses a place to go when they need help or guidance. What I love most about this site is that it has specific disorders and medications that only come up in mental health, and this allows nurses to get familiar with these disorders and medications. When nurses are more informed, patient outcomes are better and less errors are made. There is a specific tab on the site that has clear reports about problems being addressed out in the world and shows you what the outcomes are. Overall, professional nursing organizations like the APNA are crucial in uniting nurses, providing resources for professional growth, and giving access to important news regarding patient care in mental health. By allowing nurses to stay informed, and connected, these organizations play an important role in driving positive change in healthcare.

Academy of Medical Surgical Nurses is an organization for Medical Surgical nurses. They are the only national organization representing the voice of MS nurses. They are the voice at the state and federal levels. They help promote patient safety and health. They advocate for safe staffing to improve patient health and quality of care. They advocate for addressing patients by their acuity and just another number.
