Assignment: Health Promotion Plan and Evaluation Using a systematic approach, learners will develop a health promotion program plan. Learners will use a stepwise approach to ensure they incorporate key features in a systematic planning process, including determining program aim, considerations for engagement of community stakeholders/members, development of interventions to address the health need or issue, and evaluation. Course Outcomes: • Investigate interprofessional approaches to garner multifaceted resources that empower populations to attain and preserve healthy communities. • Develop strategies to influence regulatory, legislative, and public policy to advocate for healthy populations. • Examine the role of the advanced practice nurse (APN) in providing leadership in the community related to health promotion, reduction of risk of complications, illness, and injury for populations. Guidelines Learners understand that successful health promotion initiatives require strategic planning. Please include the following components in your health promotion program plan: • Begin with a brief introduction outlining the contents and purpose of the assignment • Introduce the health need or issue • Describe a framework for creating the health promotion plan. The plan should follow the framework as described. o Explain the significance of or justify why you have chosen this framework • Identify the aims or purpose of the proposed health promotion program. The aims and purpose statements should be supported with rationale from literature (e.g., Assignment 1 content) • Describe the process for community engagement in the planning process, including the advanced practice nurse role. Points of consideration can include: o Identifying essential community partnerships (e.g., who are the stakeholders and why?) o Describing the anticipated challenges and facilitators for building successful partnerships and community engagement o Describing good practices for successful partnerships that you, as the advanced practice nurse, will incorporate • In detail, describe at least two (2) interventions that will be implemented to address the health need or issue. Points of inclusion: o Designing the interventions with appropriate theory (e.g., determinants of action, communication, empowerment) o Designing feasible interventions that build on gaps in the literature or grey literature o Identifying the role of the advanced practice nurse in the interventions as well as roles of any relevant interdisciplinary/intersectoral partners • Identify anticipated challenges and strategies to consider for the implementation of a successful program and how you will address them. • Develop an evaluation plan so you, as the advanced practice nurse, can evaluate the success of the interventions and overall program. Good evaluation plans integrate SMART goals and clearly defined objectives. They are based on measures that are supported by evidence. Remember to use current and relevant resources. • Brief conclusion statement Requirements • The assignment should not exceed 8-10 pages in length (excluding the title page, references, and appendices) • The assignment should include a minimum of 10 current scholarly sources. Please strive to use Canadian sources, within the last 5-10 years unless using a classic work. Other sources such as scholarly websites, government documents/reports may also be used over and above the required journal references to supplement your work.
