In this episode, the nurse practitioner student is faced with an ethical dilemma involving a patient who is pregnant and declining support from her parent, thus claiming her legal rights as a medically emancipated minor. An ethical dilemma, according to our text, is “a situation in which an individual is compelled to choose between two actions that will affect the welfare of a sentient being and both actions are reasonably justified as being good, neither action is readily justified as being good, or the goodness of the actions is uncertain. One action must be chosen, thereby generating a quandary for the person or group who is burdened with the choice” (Butts & Rich, 2022, p. 54). The statement, “What may be an ethical dilemma for one registered nurse may not be an ethical dilemma for another registered nurse” can be related to this particular scenario in that, while Althea is comfortable continuing with a plan of care for this patient, some providers may not feel comfortable treating a minor without a parental support system in place. It may be considered unethical to prescribe medication and a plan of treatment for a patient who, if not pregnant, would be considered a minor requiring the consent of a parent.

My core values align heavily with my religious and moral beliefs: Integrity, truthfulness, excellence and hard work in all efforts, loyalty, forgiveness, extending of grace, freedom, and doing “the right thing.”
Q. Why do you value them?
These are my values because I feel both a moral obligation and a religious/spiritual impression to act in accord with these principles. It is how I believe human interactions should be conducted, how I should carry myself in the world, how I can make a difference in the lives of others. I believe in the freedom to act in a way that is representative of my values, which I believe come from a sovereign God who guides these principles and desires relationship with me (and others).
Q. What are the values in your society?
From my perspective, our society values self-determination, equality, justice, and financial success. It seems our society is quite focused on the “self” and in one’s independent journey towards success and equality, with a specific interest in women and oppressed people groups.
Q. How do your core values impact your personal choices?
My core values greatly inform my personal choices, both on a grand scale and in smaller daily decisions. My principles guide my interactions with others, what I choose to engage in or not engage in, how I spend my time and/or money. My core values impact how I interact within relationships and even whom I enter into relationship with. My value on freedom impacts my speech, how I express my religious beliefs, where I work, and how I engage with culture/society. Particularly, I choose to refrain from much of what is considered “pop culture” today in favor of more traditional, simple ways of living and more wholistic routes of wellness.
What are the limits to personal choice?
Q. Who or what limits your choices?
I would say that to an extent the government limits my choices. There are laws and rules in place which govern my professional practice, what I can do as a citizen in this country, and how to generally act in our society. In a mixed economy, there are certain choices I can make with regard to my finances and particular ways to express my economical rights as a citizen. Other social and political norms and regulations can limit the choices I make as well. Furthermore, nursing scope of practice laws limit my professional practice choices which may impact my personal choices. My financial status and inflation impacts my personal choice of selecting health providers and insurers, where I choose to live, eat, and shop.
Q. Do you find limits to choices to be beneficial or harmful? Explain why.
When involving other human life or when dealing with morality, I find limits to be beneficial. Limits can help set boundaries and prevent unnecessary harm. However, limits should be general and still allow for human choice. Human choice allows for competition, creativity, and beauty in expression that stringent limitations may otherwise hinder.
Q. Should a health care organization or the government limit people’s choices? If so, how and under what circumstances?
I think that the government should maintain general laws which limit people’s choices that could involve harm in some form towards another person or entity, but that, generally speaking, an individual’s constitutional rights should be maintained. Some of the constitution can be gray, thus governmental intervention can be helpful in interpreting the law and in detailing how it should be carried out. I think that in order to prevent a completely anarchical society, the government should provide limits in personal choice, but that too much limitation is unconstitutional. In the same way, it is fair for health care organizations to outline personal limitations and choices as they align with federal law, but not so limiting as to deny someone their right to healthcare, freedom of speech, freedom to express religion, and so on. On the other hand, health care is a business and while finances may limit personal choice for some, inflow of money is necessary to keep businesses running. With this in mind, I believe reform is absolutely necessary for our health care system, but I do not believe that the removal of all limitations (entirely free health care for all) is the answer.
