- My current clinical rotation is Park Avenue Medical LLC, a family practice in Wallingford, CT. This clinic is a family practice where we care for all ages: young adults, adults, women’s health, and geriatrics. However, most of our patients are seen for depression management and alcohol and opiate dependence. This clinic provides management for their opiate management treatment together with all other medical conditions they could be experiencing, chronic or acute. Patient demographics seen in the office are young adults to geriatrics, white Caucasians receiving treatment for opiates and alcohol dependence. The most common issue dealt with is depression, where they use Spravato while monitoring the patients in the office. Another common issue treated at this clinic is opioid and alcohol dependence. Where these patients are closely monitored before refiling their prescriptions. For example, before refilling the patient’s medication to manage opiate dependence, they have to provide a urine sample where we check if the patient has been free of any illegal drugs. If the patient’s urine comes back contaminated with any prohibited substance, they are given a one-week refill and asked to return the following week. Together with this chronic condition, we manage other medical conditions that they could be experiencing, such as COPD, heart failure, DM, etc. One of my goals for this session is to complete the requirements for the adult, women, and geriatric portion of this program. Since the last session, I have been doing pediatrics. Two peer-reviewed primary care provider-appropriate journals known for U.S guidelines and quality research articles are the American Academy of Family Physicians and the Journal of General Internal Medicine. The first site is an article about diabetes and recent research on this subject. The second site is an article about congestive heart failure and provides the latest information about this topic.
