The United States was built in part by immigrants and according to a recent study, 13.9 percent of the United States population is the immigrant population (Boruszak-Kiziukiewicz & Kmita, 2020). Many immigrants migrate to the United States in search of employment, a better education, and to improve their living standards. Migration is considered an important part of a country’s social and economic development and the United States has for years been benefiting from immigrants. However, most of these immigrants face several challenges due to several factors, such as cultural barriers, cultural differences, high illiteracy, and poverty.
They also face several health hazards that include infectious diseases, mental health issues, occupational injuries, malnutrition, lack of access to healthcare, chronic conditions, and language barriers in healthcare. For the immigrants in the case study who are farm workers, the potential health hazards they face include exposure to harmful chemicals such as extreme temperatures, pesticides, and chemicals. They also have an increased risk of injury from operating dangerous and complex farm machinery. To decrease the risk of these health hazards, J.G. should educate the farm workers on the consequences of not using protective gear such as gloves or masks when handling harmful farm products. In addition, J.G. should inform them of the benefits of reading the instructions on how to use different farm machines before using them. J.G. should also stress the importance of annual health checkups to help assess for any existing conditions, which will help improve their overall health.
- Discuss how you would advise young adults to select contraceptive methods. Do you have any personal, religious, or other reservations or discomforts that would interfere with your ability to advise clients and ask them how they would address these personal conflicts?
It is often difficult to select a type of birth control due to the wide variety of options available (Morimont et al., 2021). When advising young adults on the type of birth control methods, it is important to advise them to consider factors such as effectiveness, convenience, desire to have children in the future, and frequency of sexual activity. Contraception devices, such as hormonal intrauterine devices, are considered safe and effective and do not require much effort from the user. If young adults desire to get pregnant in the future, birth control methods such as sterilization are not recommended, and they should instead consider a reversible method such as a vaginal ring or a barrier method such as condoms. In addition, they should choose a birth control method that suits their lifestyle. For example, the IUD is easy to use and convenient. They should also consider their tolerance to side effects and choose those with the fewest side effects, such as the IUD. I believe contraception is important as it helps protect against unintended pregnancies, sexually transmitted infections, and unsafe abortions. As such, I do not have any personal or religious reservations or discomforts that would interfere with my ability to advise clients about contraception. However, advising the clients to take into consideration their personal and religious beliefs before choosing a birth control method is important.
- Have students develop a smoking cessation plan for a client.
There is no exact way to stop smoking tobacco but there are several important steps that can help a person’s decision to quit be successful (Shang et al., 2022). The first step is to decide to quit and write down the reasons why they want to quit. Next, the person should set a date for their quit date and they should not pick a date that is too far as it gives them time to change their mind or too close as they need time to prepare. The person can pick a date that has a special meaning, such as a birthday or anniversary. The next step is to plan how they will quit, such as through nicotine replacement therapy or prescription drugs. The person should also plan for support, as this is an important step in the cessation process. The person should then prepare for the quit date, such as by marking the day on the calendar, informing friends and family, getting rid of all cigars and ashtrays, and stocking up on oral substitutes such as sugarless gum and carrot sticks. Before quitting, the person should cut back on how much they use and cut back on when and where to use it. On the quit day, the person should stay busy, such as through physical activity and exercise, not use tobacco, not even one puff, and drink lots of liquids. Attend a quit class for support and guidance; avoid people and situations that trigger the desire to smoke.
- Evaluate the impact of poverty on older adults. Make sure to include the impact on their physical and mental health and health promotion recommendations.
According to the Census Bureau, approximately one in seven elderly people aged 65 and older has income that is below the poverty threshold (Ma et al., 2022). Poverty affects the health of the elderly negatively, as some of them are dealing with chronic conditions. Even though they can rely on Medicare benefits for hospital visits, they are unable to pay for medicines and other recommended procedures. This negatively affects health outcomes and their quality of life. In addition, they are at risk for obesity and cardiovascular diseases because, without enough income, they have to depend on cheap options that are high in calories. Poverty also affects the mental status of the elderly due to increased stress, depression, and anxiety. Some of the health promotion recommendations include improving access to affordable and nutritious food, such as by providing this population with referrals to food assistance programs such as food banks or government nutritional programs. There are many health hazards associated with the migrant population in general and the one referred to in the case study. A list of these health hazards includes:

- Mental health challenges
These health hazards are associated with low economic status commonly encountered by migrant populations. This population may struggle to afford healthcare services, including preventative care like vaccines. They may lack health insurance and face financial barriers that delay needed treatment. Migrants with a low economic status may struggle to afford nutritious foods and rely on cheap, unhealthy options. This can lead to health issues such as obesity, diabetes, and nutrient deficiencies. Poor living conditions can increase the risk of infectious diseases and contribute to overall poor health. Migrant workers are exposed to workplace hazards such as working with heavy machinery or exposed to hazardous chemicals. Migrants are also at increased risk for exposure to violence including physical or sexual violence, human trafficking, and exploitation. All these health hazards can cause mental health challenges such as anxiety, stress, depression, and thoughts of suicide.
Recommendations to the migrant population to prevent injuries related to those hazards would include providing resources and education for them. Nurse practitioners can provide resources that help with accessing healthcare services. Nurse practitioners can help migrants enroll in health insurance programs and help them find affordable healthcare services. Health education should be given to these individuals to discuss preventative care, nutrition, hygiene, and infectious disease prevention. Migrants should be taught about their rights in their workplace and to report workplace hazards or injuries. If an individual is facing any type of violence, the nurse practitioner can provide resources for support for migrants that include legal services and safe housing options. An interpreter can be used during this teaching to ensure effective and sensitive communication.
Contraceptive Methods for Young Adults
Nurse practitioners need to advise young adults in selecting contraceptive methods. I do not have any personal religious reservations or discomforts that would interfere with my ability to advise clients on selecting contraceptive methods. The nurse practitioner should first assess the individual’s medical history and contraindications to certain contraceptive methods. Education would be provided on contraceptive options including emergency contraception options. All concerns and preferences would be addressed regarding contraceptive methods. The patient’s choice of contraception will decide the plan of care. The nurse practitioner will educate the patient on the contraception option, discuss preventing sexually transmitted infections (STIs), and follow up with the patient.
Smoking Cessation Plan
Developing a smoking cessation plan for a client would include performing an assessment. The client’s smoking history needs to be obtained to understand the frequency of smoking, how much is smoked a day, how long the client has smoked, previous attempts at quitting, and triggers for smoking. The client’s readiness to quit needs to be assessed and encouraged. A date should be set with the client to quit smoking. Information can be provided that explains the benefits of smoking. The combination of behavioral therapy, such as motivational interviewing, and pharmacotherapy (nicotine replacement therapy, varenicline, and bupropion) can double or triple longer-term abstinence rates and should be offered to all patients (Lampridou, 2023). Coping skills can be identified to help the client with cravings. Support groups should be offered to encourage other individuals who have stopped smoking. Follow-up appointments need to be scheduled to monitor the client’s progress.
Impact of Poverty on Older Adults
Poverty impacts older adults by limiting their access to healthcare. This impacts their access to preventative care, current care, medications, and healthcare services. Older adults who cannot afford their medications will experience poor health outcomes. Higher rates of chronic diseases are associated with poverty such as diabetes, hypertension, heart disease, and arthritis. Poverty also affects the nutritional status of older adults which can cause malnutrition, obesity, and nutrient deficiency. The financial burden and limited access to healthcare cause mental health issues in older adults. The nurse practitioner should provide healthcare resources so older adults can use programs that assist in providing healthcare services. Resources for food insecurity can be set up for these individuals such as Meals on Wheels who would deliver food to their homes. Transportation services can be arranged to take older adults to their appointments. Mental health services can be arranged through telehealth services, community-based clinics, and outreach programs.
