Nurse managed health centers and clinics are becoming increasingly popular as we strive to provide health care access to more Americans. The demand for physicians is much too high for the supply, hence advanced practice nurses are stepping in to address the gap. Per the Affordable Care Act that was passed during former President Obama’s administration, the goal to provide every American with health insurance and access to care is an ambitious yet challenging goal. If nurse managed clinics were not allowed, there would be many more people unable to get access to care and the goal would be far from being met. The benefits of NP clinics are an ease in physician workload and increased access to care for Americans.
Obstacles to nurse managed clinics include public distrust of NPs, laws and policies limiting NP privileges, and physician pushback (Firth, 2023). Unfortunately, many patients prefer physicians instead of advanced practice nurses because they have more trust in doctors. They believe that because doctors have more education than NPs, that NPs will make more mistakes and not diagnose/treat them correctly. However, according to recent research, NPs and doctors have virtually identical error rates (Firth, 2023). NPs are just as capable of diagnosing and treating patients as doctors are, with the only real difference being doctors are able to perform surgery. In addition to public distrust, there are laws that limit the NP’s privileges. In most states in the US, NPs cannot practice independently. Another obstacle NPs face is physician pushback. The American Medical Association- a professional organization that represents doctors and medical students, strongly opposes NPs practicing without physician supervision- an act they label as scope creep (Firth, 2023). They claim that it is more dangerous for NPs to practice independently, however research shows very similar error rates amongst NPs and doctors. Solutions to these obstacles are to educate the public on NP’s capabilities, and to communicate with the AMA and lawmakers the research showing NPs do not make more errors.
