Working in the postpartum area, I encountered an ethical dilemma concerning the management of neonatal abstinence syndrome (NAS) in newborns born to mothers with a history of substance abuse. NAS occurs when newborns experience withdrawal symptoms due to exposure to addictive substances in utero, presenting nurses with complex ethical considerations regarding the care of both the mother and the newborn.
The ethical dilemma arose when a newborn born to a mother with a history of opioid addiction exhibited signs of NAS shortly after birth. As a nurse, I faced conflicting obligations—providing compassionate care to the newborn while supporting the mother’s recovery. Additionally, I was faced with questions about the appropriate level of intervention and the balance between promoting maternal autonomy and safeguarding the well-being of the newborn.
The first step in addressing the ethical dilemma was conducting a comprehensive assessment of the newborn and the mother. We engaged in collaborative discussions to explore the ethical dimensions of the dilemma and develop a holistic care plan that considered the needs of both the mother and the newborn (Haarh et al., 2019).
Accepting a harm reduction approach, I focused on mitigating the potential harms associated with NAS while supporting the mother in her recovery. This involved providing nonjudgmental support, education, and resources to the mother, including access to substance abuse treatment programs, counseling services, and support groups (Haddad & Geiger, 2023).
I engaged in open and transparent communication with the mother, providing information about NAS, its potential consequences, and available treatment options. I emphasized the importance of informed decision-making and encouraged the mother to actively participate in caring for her newborn while seeking support for her recovery (Haddad & Geiger, 2023).
Recognizing the importance of family-centered care, I involved the mother and other family members in caring for the newborn, adopting a supportive environment, prioritizing bonding, and nurturing relationships. By involving the family in the care process, I aimed to promote the well-being of both the newborn and the mother while respecting their autonomy and dignity (Haarh et al., 2019).
I provided ongoing monitoring and support to the mother and the newborn throughout the postpartum period. This included regular assessments of NAS symptoms, tracking of maternal progress in substance abuse treatment, and provision of emotional support and counseling to address any challenges or concerns that arose (Haddad & Geiger, 2023).
In conclusion, ethical dilemmas in postpartum nursing, such as the management of NAS in newborns born to mothers with a history of substance abuse, require nurses to navigate complex moral environments with compassion, empathy, and ethical integrity.
