Ethical Dilemma
When two or more principles or values disagree and there are mutually inconsistent courses of action, this is referred to as an ethical dilemma (Giannetta et al., 2021). Nurses are faced with daily demands from medical teams and hospital management, while also caring for patients and families. Nursing involves taking a holistic approach and they prioritize the individual’s dignity and integrity. Nursing involves ethical and moral concerns as nurses make moral decisions daily, including routine issues and life-threatening situations (Haahr et al., 2019).
There are several types of ethical dilemmas that nurses face in practice. Some of these include honesty versus withholding information, science versus spirituality, healthcare needs versus resource allocation, and autonomy versus beneficence (Duquesne University’s School of Nursing, 2020). While practicing as a registered nurse, I have faced ethical dilemmas and have had to navigate cautiously in how I approach the situation. I work in the pediatric oncology and hematology department, where many families are provided difficult information to take in, such as their child facing a cancer diagnosis. This kind of information can be very daunting for families as it changes their lives completely and sometimes comes as a complete surprise. Additionally, this information is not only difficult for families, but it is especially difficult for the patients. Explaining to children that they now have cancer and will have to undergo extensive treatment and hospitalizations is not an easy task. There are different ways to approach this based on the child’s age and developmental level.
A prime example of an ethical dilemma that I have encountered during my practice includes honesty versus withholding information. As I mentioned previously, a cancer diagnosis can be very heavy news for a child and family to experience. Therefore, families may want to withhold this information from their child to avoid scaring them and causing additional stress. Although it is burdensome information for the child and may raise many questions and concerns, it is never ethical to withhold a diagnosis from the child. Patients have the right to learn about their medical conditions. Choosing how to share this information, especially if it contradicts the family’s beliefs, can be a difficult decision. It is essential for the nurse to be honest with the patient to establish a trusting relationship. Nurses must understand that trust is the basis of the nurse-patient relationship.
In these ethically difficult situations, nurses must make decisions based on bioethical principles such as autonomy, beneficence, nonmaleficence, and justice. Respect for autonomy translates to each individual’s personal values and beliefs, as well as his or her right to make their own treatment and care decisions based on their preferences. Nurses must ensure that patients can make informed decisions and act on the information they are provided with. Moreover, beneficence refers to actions that are beneficial to others, whereas nonmaleficence is a responsibility not to cause harm to others. All actions and decisions regarding patients’ treatment and care must therefore be based on a thorough evaluation of what is most beneficial to the patient, while keeping the patient’s interests in mind. The concept of justice signifies ethical decision-making, which is based on fairness and equality. Justice entails adhering to universal rules where respect for autonomy, objectivity, and a positive mindset are of upmost importance (Haahr et al., 2019).
