SCENARIO Today you are meeting with Garry. He is 55 years old. He was managing a team of 10 people. He had lots of responsibilities. Garry had worked in his job for the last 15 years. He moved into the role after already having worked 22 years for the same employer. Garry began working for the company straight after school and has never worked for anyone else. After 37 years, the company restructured and Garry was no longer required. He does not want to retire. His family recently purchased a small farm just outside the city. They are still paying the mortgage. His wife has a part-time job but it is not enough money to support them. Garry and his wife hoped to retire on the farm. Now Garry is worried they will have to sell the farm. Garry says that he lies awake at night dreading what will happen next. Garry has been looking for further employment but has not had any luck. He is very concerned that he will not be able to find any work at his age. Last week, Gary had a panic attack when shopping with his wife. He felt tight in his chest and had trouble breathing. His wife called an ambulance as she thought that Gary was having a heart attack. Gary later told hospital staff that he became extremely anxious when he saw the total bill for the groceries his wife was buying. He began wondering how long he could afford to pay that amount if he didn’t find work. TASK Throughout the role play, make sure that you: • Are taking into account the key principles of person centred approach and demonstrate its skills: o Effective use of body language. o Reflecting feelings and content. o Open and closed questioning. o Paraphrasing. o Rapport building. o Summarising. • Listen carefully to your client and show him empathy. • Make your client feel safe, don’t judge him and don’t give him any advice. Observe and take into account your client’s body language and respond accordingly demonstrate the use of CBT specific skills/techniques. For example: o Challenging negative belief. o Reframing. o Decatastrophising. o Re-attribution. In the role play, make sure that you have covered the following tasks: • Select counselling therapies: 1. Identify and consider any co-existing issues Garry, your client is experiencing. 2. Incorporate your analysis of the client’s developmental status and response to change. 3. Select courses of action in working with the client. 4. Clearly communicate details of therapies to be used with your client using language the client understands. Use counselling therapies: 1. Demonstrate how you will work with the client using Cognitive Behaviour Therapy (Use the techniques according to the principles and application of the therapy). 2. Combine at least one technique or process from one of the other five counselling therapies you have studied. 3. Interact with the client with empathy, sensitivity, professionalism and courtesy. 4. Use appropriate counselling skills and techniques (Hint: Person Centred Therapy skills: including empathic listening skills). 5. Use appropriate verbal and non-verbal communication approaches that acknowledge the client’s emotional needs. Offer support and information: 1. Provide your client with information about support services and resources and refer them to options for further help as needed. 2. Identify, suggest or use strategies for formal and informal grief support. 3. Obtain feedback from the client to confirm that options are clearly understood
