This week was yet another one of its intriguing natures. Once more, I cannot fathom that this clinical rotation is ending. The past week has sped by, and with each passing day, my proficiency in assessment and diagnosis has grown. I have complete faith in my abilities to think critically. A 67-year-old male patient presented to our facility this week exhibiting signs and symptoms including postnasal drip, sputum production, cough, congestion, 100.5F fever, and a sensation of fullness in both ears. He reports that he started to feel ill two days ago; however, as of today, swallowing caused crackling sounds in his ears, and he felt exhausted overall. When queried about the hue of the sputum, he reported observing that it had a yellowish-green hue. He began to feel exhausted and feverish this morning. The patient reported experiencing sinus tenderness around his nostril and forehead upon examination. An examination of both middle inner ears revealed redness and inflammation, and the patient reported mastoid bone tenderness. He stated that he took a few Tylenols for the pain yesterday and has been consuming steaming tea, neither of which have been particularly effective in alleviating his symptoms.
Following the completion of the patient assessment, it was determined that the patient was diagnosed with acute rhinosinusitis. According to Aring & Chan (2016), symptoms of acute rhinosinusitis manifest when the mucosal lining in the paranasal sinuses and nasal cavity becomes inflamed. Because the nasal mucosa is contiguous with mucosa of the paranasal sinuses, inflammation of the sinuses rarely occurs without inflammation of the nasal mucosa. Although this process is commonly called sinusitis, rhinosinusitis is the more accurate term. Each year in the United States, rhinosinusitis affects one in seven adults, resulting in more than 30 million annual diagnoses. Rhinosinusitis is the fifth most common diagnosis for which antibiotics are prescribed in the United States. The most effective treatment for this diagnosis would be 500 mg of Ciprofloxacin orally every 12 hours for ten days. The infection will be treated with this. The patient has been informed of the critical nature of finishing the full course of antibiotic treatment for ten days to eradicate the infection. The patient was additionally prescribed Azithromycin for a duration of five days, along with an intramuscular injection of Rocephin to mitigate inflammation and congestion.
