BP is a 25-year-old 73 kg, 5’9″ male renal transplant patient who is 4 months post-surgery. He has been on 300 mg every 12 hours of Neoral®. His Css,avg concentration is 1550 ng/mL, assume he is at steady state and that CL and F remain constant. The team would prefer a Css,avg concentration closer to a target of 1100 ng/mL. Please suggest a new regimen for BP of Neoral®. Recommend ______mg of Neoral® every 12 hours. (round to the nearest 100 mg) The expected Css,avg when on this regimen is ______ ng/mL (round to the nearest whole ng/mL) In order to avoid concentration related side effects we would need to hold the Neoral to allow the concentration to fall.
