Task 4: Case Presentation: – Analyse client health findings and plan services Select any client under your care. It is preferable if you select clients who have been hospitalized for more than two (2) days and you have cared for them on two (2) days. Review their Lab test results, investigations and observations (vital signs, Fluid balance chart, bowel chart, BGLs, Neuro- observation, Neurovascular observation). Review client’s plan of care- on admission and current. Present the client’s case verbally to your assessor/ clinical supervisor personally or in a group. In your presentation, address the following: a) List the diagnosis for the client (from history and reason for current admission and discuss briefly related anatomy, physiology, and pathophysiology of the diseases. b) Identify the observations which are out of the normal range. c) Discuss the rationale for the change in observation and the actions taken for this client to address the concern. d) Identify the investigation/ sample test results that were abnormal. e) Discuss the rationale for the change in test results and the plan of care for this client to address the concern. f) Discuss with team/ RN/ supervisor if there were any conflicts in the plan of care advised
