Week 4 Discussion 1
What are some of the obstacles or barriers to implementing EBP in nursing? Provide a rationale for your answer. Since there are numerous topics on the issue, it is not appropriate to repeat one that has already been mentioned unless providing new information.
Week 4 Discussion 2
Conducting research projects while ensuring the protection of human subjects is necessary. What are some methods to protect personal rights of someone in one of the groups that is labeled as vulnerable? Justify your rationale.
NUR 699 GC Week 4 Assignment
Evidence-Based Practice Proposal – Section D: Solution Description
Details:
Write a paper of 500-750 words (not including the title page and reference page) paper for your proposed evidence-based practice project solution. Address the following criteria:
- Proposed Solution: (a) Describe the proposed solution (or intervention) for the problem and the way(s) in which it is consistent with current evidence. Heavily reference and provide substantial evidence for your solution or intervention. (b) Consider if the intervention may be unrealistic in your setting, too costly, or there is a lack of appropriate training available to deliver the intervention. If the intervention is unrealistic, you may need to go back and make changes to your PICOT before continuing.
- Organization Culture: Explain the way(s) in which the proposed solution is consistent with the organization or community culture and resources.
- Expected Outcomes: Explain the expected outcomes of the project. The outcomes should flow from the PICOT.
- Method to Achieve Outcomes: Develop an outline of how the outcomes will be achieved. List any specific barriers that will need to be assessed and eliminated. Make sure to mention any assumptions or limitations which may need to be addressed.
- Outcome Impact: Describe the impact the outcomes will have on one or all of the following indicators: quality care improvement, patient-centered quality care, efficiency of processes, environmental changes, and/or professional expertise.

What are some of the obstacles or barriers to implementing EBP in nursing?
Introduction
Evidence-based practice (EBP) is a process aimed at improving patient outcomes by using evidence from clinical studies. Evidence-based nursing (EBN) is the application of EBP, in particular, to nursing practice. While some nurses may already be familiar with the term “evidence-based medicine” or even “evidence-based healthcare,” far fewer have had any contact with “evidence-based nursing.” In this article, I’ll describe some of the key barriers and obstacles that prevent nurses from incorporating EBP into their practices.
The time required to conduct research and implement the findings from evidence-based practice.
One of the biggest challenges for EBP is that it takes time to implement. Research has been done on this topic, and one study found that it took 17 years for an intervention to become widely accepted by nursing practice. This means that if you want your nurses or residents to use EBP in their clinical work, they need time lots of it!
How much time will it take? According to another study conducted at Johns Hopkins University School of Nursing, implementing evidence-based practice across all aspects of patient care will require approximately four years (from start to finish).
The difficulty of putting the practices required for evidence-based nursing into place.
EBP is a process that requires time, patience and commitment. The first step is to formulate a plan of action based on the findings of the literature review and practical application to your practice. Then you need to implement this plan by using what’s called “the iterative approach”—that is, you take small steps forward toward your goal until it becomes more ingrained in your daily life as a nurse.
It may seem like an insurmountable task at first glance but there are ways around this obstacle:
-
Start small – don’t try to change everything at once! This can be overwhelming so start with just one or two changes that will help improve patient care outcomes (or whatever else) within your department/practice setting(s). For example: Maybe one day instead of ordering medication off-site because you’re short staffed on weekends or during lunch breaks (which could lead up complications), perhaps instead order it directly from pharmacy through e-mail or text message? Or maybe even just bring our own pill bottles so patients don’t have access issues while they’re out sick?”
The high costs associated with implementing certain evidence-based practices.
The high costs associated with implementing certain evidence-based practices.
Estimates of the cost of implementing EBP vary widely, but most experts agree that it can be very expensive. For example, in one study of pediatricians’ use of EBP in practice management and decision making, researchers found that implementing these best practices would require $9 million per year for training and equipment (Wolfe et al., 2003). In another study examining the implementation costs associated with quality improvement programs across health care settings, researchers found that because each program is unique and often needs to be customized for each patient population being served by the organization or institution where it’s implemented (Kreuter & Nelson 2008), there may be additional expenses involved at different points along this pathway as well for example: paying salaries/benefits; setting up infrastructure such as computers; purchasing software licenses; upgrading technology used within an organization’s networked environment (including communication systems); hiring additional staff members who will oversee these initiatives; etcetera…
Whether a study’s findings will be applicable to their own clinical settings, and their ability to determine which studies are reliable.
You will need to conduct your own research and make sure it is applicable to your setting.

The quality of the research study can often be determined by looking at the methodology and design. Research studies use a variety of approaches to answer research questions, including surveys, interviews, experiments and case studies. These different types of studies all have strengths and weaknesses that should be considered when evaluating the findings.

-
The need to balance the needs of patients with the need to do evidence-based practice.
-
The importance of having time to do research and implement findings (e.g., new medication or treatment options).
-
A team approach should be used when implementing an EBP model because it makes it easier for all involved parties (i.e., physicians, nurses) as well as staff members who may not be directly involved in patient care but whose input is essential in order for a successful outcome at each step along this journey. It also provides support from other professionals within your organization such as social workers or mental health providers if needed due diligence needs might require outside assistance from them depending on what stage(s) you’re currently working on at any given moment—and remember that these steps aren’t necessarily sequential either! Some people might find themselves skipping over one step altogether depending on what stage they’re currently working through; however…
The main barriers are time and cost, but efforts are underway to overcome those issues
The main barriers to implementing EBP in nursing are time and cost, but efforts are underway to overcome those issues. These include the use of e-learning, webinars, and online courses. For example:
-
A course that provides a comprehensive overview of how EBP can be used as a tool for improving patient outcomes. This course may be offered as an online certificate program or seminar and costs around $1,500-$3,000 per student (depending on number of credits).
-
An ongoing collaboration between providers who provide care for patients with mental health issues; researchers studying this area; healthcare managers who manage services provided by these providers; clinicians who treat patients with mental health issues; educators working within schools where students learn about mental health issues (i.e., high schools); etcetera…
Conclusion
Implementation of EBP is a critical component to ensure the best outcomes for patients. Nurses should use evidence-based practice as a guide to their decisions, and make sure they’re using the best practices possible for their patients. When implementing this model, nurses need to be aware of potential barriers so that they can overcome them if necessary.
