In 500-750 words (not including the title page and reference page), apply a change model to the implementation plan. Include the following:
- Roger’s diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck’s change curve model or the transtheoretical model of behavioral change. Other conceptual models presented such as a utilization model (Stetler’s model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based intervention in clinical practice.
- Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
- In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 5 with the narrative, the conceptual model should be placed in the appendices for the final paper.
Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.
Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.
Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.
The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

Introduction
There are many different models for change, and the Transtheoretical Model is just one of them. It was first introduced in 1979 by Prochaska, DiClemente, and Norcross to help explain why people don’t want to change their behavior. The theory goes like this: People go through four different stages before they decide to make a change in their lives—precontemplation (where they’re not even thinking about making a change), contemplation (thinking about what it would take), preparation/determination (taking steps toward changing), action (doing something concrete to move forward). Once the person has achieved some level of success with their goals, they’ll encounter relapse back into one of those other stages depending on how well they’ve progressed.
Theory of Transtheoretical Model
The theory of transtheoretical model is a model used to help people understand how they can change their behaviors. This model was created by Marlatt and Gordon, who wanted to explain why some people try drugs, while others do not. The theory says that individuals have different stages in the process of changing their behaviors:

Precontemplation – individuals may not want to participate in any form of drug use but they’re not ready or willing to change at this time;

Contemplation – this stage involves thinking about whether or not one wants to make these changes;

Preparation – this is when people start taking small steps toward changing their behavior (for example, going cold turkey from heroin).
Precontemplation
As you might expect, precontemplation is the first stage of the model. People in this phase have no intention to change and they’re not aware of the need to change. For example, if your friend is overweight and hasn’t been working out regularly since high school but hasn’t given it much thought lately because he’s busy with work and other activities, he may still be precontemplative when asked about his fitness level: “I’m not sure I want to get into shape,” or something similar.
Contemplation
Contemplation is the stage where you are thinking about change, but not doing anything. You may be in this stage for a long time and not sure if it will work. You may be thinking about what you have to lose if your behavior changes for example, “I won’t get my promotion this year because I stopped smoking” or “My family will disown me forever if I stop drinking alcohol every night.”
Preparation/Determination
This is the stage in which people are prepared to make a change. They have made a decision to change and are motivated to do so. They also have a plan for implementing their new behaviors, such as setting goals and making lists of what they need to do along the way.
Action
Action is the stage where you are applying what you learned in the previous stages. You are taking action to change your behavior, and this is a time when it’s important not to fall back into old habits or make changes that will negatively impact your desired outcome. For example, if you want to be more productive at work but find yourself constantly distracted by emails, then maybe it would be better for you not to check email at all during work hours instead of just checking them occasionally (which could lead back into old habits).

The maintenance stage is the longest and most difficult to change. It is also the time when you are trying to keep your new behavior in place. You will be tempted to go back to your old behavior, but this time around, it’s important that you don’t let yourself do so. You need to keep your mind focused on your goals and develop a new habit a good one!
Relapse
Relapse is a normal part of the process. It’s not failure, but instead it’s a way to learn more about yourself and your habits.
Relapses are not the end of the world they’re just steps backward on your journey toward change. You can get back up again and keep going with whatever you want to do next!
