NSG6440 Week 4 Discussion Paper
This week’s content addressed common chronic diseases. Please review the case study below and answer the following questions:
A sixty-year-old baker presents to your clinic, complaining of increasing shortness of breath and nonproductive cough over the last month. She feels like she can’t do as much activity as she used to do without becoming tired. She even has to sleep upright in her recliner at night to be able to breathe comfortably. She denies any chest pain, nausea, or sweating. Her past medical history is significant for high blood pressure and coronary artery disease. She had a hysterectomy in her 40s for heavy vaginal bleeding. She is married and is retiring from the local bakery soon. She denies any tobacco, alcohol, or drug use. Her mother died of a stroke, and her father died from prostate cancer. She denies any recent upper respiratory illness, and she has had no other symptoms. On examination, she is in no acute distress. Her blood pressure is 160/100, and her pulse is 100. She is afebrile, and her respiratory rate is 16. With auscultation, she has distant air sounds and she has late inspiratory crackles in both lower lobes. On cardiac examination, the S1 and S2 are distant and an S3 is heard over the apex.
- What is the chief complaint?
- Based on the subjective and objective information provided what are your 3 top differential diagnosis listing the presumptive final diagnosis first?
- What treatment plan would you consider utilizing current evidence based practice guidelines?
Submission Details:
- Post your response to the Discussion Area by the due date assigned. Respond to atleast two posts by the end of the week.
The discussion assignment provides a forum for discussing relevant topics for this week based on the course competencies covered.
For this assignment, go to the Discussion Area and post a response to one question in the Discussion Area by the due date assigned. You may respond to your classmates’ posts for either question.
To support your work, use your course and text readings and also use the South University Online Library. As in all assignments, cite your sources in your work and provide references for the citations in APA format.
Start reviewing and responding to the postings of your classmates as early in the week as possible. Respond to at least two of your classmates’ initial postings. Participate in the discussion by asking a question, providing a statement of clarification, providing a point of view with a rationale, challenging an aspect of the discussion, or indicating a relationship between two or more lines of reasoning in the discussion. Cite sources in your responses to other classmates. Complete your participation for this assignment by the end of the week.
As a family nurse practitioner you are working in a rural health clinic. You are evaluating a 16-year-old adolescent patient who comes in complaining of having a difficulty concentrating in school. On exam you also note that the patient is very thin and frail in appearance and is asking you for diet pills.
What are some initial areas for concern? What screening tools can help lead you closer to your diagnosis?
Describe 1 health promotion strategy you can discuss with the patient.
Be sure to address the following in your plan of care: pharmacological and non-pharmacological (OTC) interventions, labs, follow-up, teaching, and referral/s.
Your work should integrate course resources (text/s) as well as a minimum of two (2) other evidence-based guidelines and/or articles published within 3-5 years.

Common chronic diseases
Introduction
There are many chronic diseases that can affect your health, and it’s important to know about them. You may not have them yet, but the chances are that you will at some point in your life. Here’s a rundown of some of the most common ones:
Diabetes
Diabetes is a chronic disease that affects the way your body uses sugar (glucose). It can be either type 1 or type 2, which are both caused by the same thing: insulin resistance. Insulin is a hormone that helps transport glucose from your blood into cells of your body. When there isn’t enough insulin available, glucose builds up in the bloodstream, causing high levels of glucose to return back into the body in an effort to get rid of it quickly. This process causes ketones and acidosis – another name for “acidosis” means having too much acid in your body; this condition leads to many other health problems such as heart disease and stroke due to high blood pressure/heart issues due to having too much calcium in their arteries (think gout).
Type 1 diabetes occurs when someone doesn’t produce any insulin at all because their body has been damaged by viruses like rubella or cytomegalovirus (CMV) during childhood through adulthood; while type 2 diabetes occurs when someone produces less than normal amount of insulin over time because their pancreas has stopped working properly due to obesity/poor diet choices like eating processed foods instead of whole foods which contain fiber which helps keep constipation away so they don’t feel sick enough not eat anything else besides drinking water until morning comes around again!
Heart disease
Heart disease is the leading cause of death in the United States and represents a major public health concern. Heart disease can be deadly if left untreated, but there are steps you can take to prevent it from happening to you.
Heart attacks are caused by plaque buildup in your arteries that blocks blood flow to your heart muscle. This causes swelling, pain and difficulty breathing due to oxygen deprivation (heart attack). Strokes happen when part of the brain becomes damaged or bruised by the blood vessel wall rupturing due to plaque buildup on its surface or within it; this causes damage that may result in paralysis or death if not treated quickly enough!
High blood pressure
High blood pressure is a common condition. It’s most often caused by age, obesity, smoking, being overweight and a family history of high blood pressure.
Symptoms include headaches, dizziness and blurred vision. Treatment includes medication and lifestyle changes such as eating healthier foods and exercising regularly (at least 150 minutes each week).
Parkinson’s disease
Parkinson’s disease is a neurodegenerative disorder that causes problems with movement. It is characterized by the degeneration of neurons in the brain, which result in tremors and rigidity. Symptoms may be mild to severe and vary from person to person depending on their age at diagnosis; many people experience some degree of motor impairment before they begin experiencing symptoms like shaking or rigid muscles.
It’s important for people with Parkinson’s disease to know about this common condition so that they can take steps towards minimizing its effects on their overall health and quality of life
Stroke
Stroke is a serious medical condition that can cause permanent damage or death. A stroke occurs when the blood supply to part of the brain is interrupted, resulting in brain cell death. There are two types of strokes: ischemic (or “clot-related”) and hemorrhagic (or “bleeding”).
Ischemic strokes occur when a blood clot blocks an artery supplying oxygen-rich blood to parts of your brain. Hemorrhagic strokes occur when bleeding causes blockages within arteries or veins leading to your brain’s tissue. The risk of stroke increases with age, but it can occur at any age.
There are many ways you may be at risk for having an ischemic stroke:
Arthritis
Arthritis is inflammation of the joints. It can be caused by injury, infection, or genetics. Symptoms include pain, stiffness and swelling in your joints (called morning stiffness), loss of function in your joints (called morning stiffness), as well as breathing difficulties due to restricted movement around your neck and shoulders.
There are five types of arthritis: osteoarthritis (degenerative joint disease), rheumatoid arthritis (inflammatory joint disease), juvenile idiopathic arthritis (in children aged under 16 years old) psoriatic arthritis (in people with a skin condition called psoriasis) gouty arthritis caused by excess uric acid buildup in blood vessels that leads to painful attacks
Cancer
Cancer is a group of diseases that start in cells, which are the basic building blocks of tissues and organs. The word “cancer” comes from the Latin word carcinoma, which means “an abnormal growth” or “a mass.”
Cancers can be found in many different parts of your body and affect any tissue type, including:
-
Breast cancer (cancer that starts in breast tissue)
-
Colorectal cancer (cancer that starts in the lining of your bowel)
-
Endometrial cancer (cancer that starts inside the womb)
There are several types of cancers:
Chronic obstructive pulmonary disease
COPD is a progressive lung disease that makes it hard to breathe. The main cause of COPD is smoking, but it can also be caused by breathing in toxic fumes or other irritants.
COPD affects about 18 million people in the U.S., making it the leading cause of death from chronic lung disease among adults over age 55 and teens under 25 years old.
Takeaway:
The takeaway from this article is that, if you want to live a long and healthy life, it’s important to take care of yourself. Make sure you get enough sleep, exercise regularly and eat a balanced diet.
