Answer – The focus of my leadership project is to lower readmission rates in the bariatric surgical unit. The goals I set for myself are: Reduce nausea and vomiting, reduce constipation, reduce dehydration, and reduce post op bleeding. The other goal is to redefine the nurse navigator role. The nurse navigator role has been redefined as follows:
• Will contact the patient once the patient has been scheduled for surgery.
• Will meet with patient during preop to introduce herself to the patient and their support system (if they came).
• Will meet with the patient during intraoperative to ensure that the patient is okay and to answer any questions the patient may have. She will let the patient and their support system know that there will be an educational packet waiting for them in their room. She will ask them to read over the material and write down any questions they may have.
• Will meet the patient postop in the patient’s room to start the educational portion of the process even though the patient must go through educational classes before the surgery. This will ensure that the education is reiterated to the patient. This will give the patient and their support system time to understand what is going on and remind them that they chose to take this journey.
• Will visit patient everyday while they are in the hospital and answer any questions they may have.
• Will contact the patient 24-48 hours after they go home to see how they are doing. Help them if they are having any problems with nausea, vomiting, constipation, dehydration, or bleeding.
• Will continue to contact the patient until the patient goes for their postop appointment with the doctor.
2. How did you discover this need for this project, and how did it fit into the organization’s philosophy and mission?
Answer – The project was discovered because my preceptor was given this unit. This unit is failing in productivity. The organization philosophy and mission are about advance the healthcare. This project fits into the organization’s philosophy and mission because they are trying to build up the New Start program. This program at Memorial Hermann Hospital helps patients to change their lives for the better.
3. Did you encounter any real or potential barriers during your practicum experience related to your project?
Answer – The potential barriers for this project are the doctors needing a change. The doctors do not like to change and with this project there will have to be changes made in order for the readmission rate for bleeding is under control. Once the patient is set up for preop there should be standing orders and an updated history and physical (H&P) that should accompany the patient to their preop. The H&P should have an updated list of medications that the patient has, and if the patient is on blood thinners it should be noted in the H&P that the patient may have a potential for bleeding after surgery. If the doctor does not order bloodwork for the patient that is on blood thinners the preop nurses should have standing orders so that the patient get the correct hemoglobin and hematocrit numbers before the patient has surgery. The other barrier is the nurse navigator position. This position has to have a nurse that wants to be compassionate with these patients and wants to be interested in the patients’ lives. This is what it will take for this program to be successful.
4. Discuss how the outcomes of your project will be measured. Is your project one that is sustainable, or is it designed to be a single occurrence? What actions would be needed to ensure project sustainability?
Answer – The outcome of this project will be measured through MBSAQIP. MBSAQIP is a website dedicated for bariatric patients. This project is not a single occurrence but will be sustainable by my preceptor. The actions that are being taken are to have the nurse navigator makes a chart of the patients and to follow the patients pre, intra, and post. Also, to have a weekly meeting with the nurse navigator to discuss the patients and any potential issues that may arise.
5. Evaluate the success of your project. If you were not able to implement your project, discuss why. What are the next steps for the project?
Answer – The project has already been implemented. The nurse navigator is the key to this project. She is the first person to contact the patient after the patient is scheduled for their surgery. She will make sure continual of care is started from the patients preop to the patient going home to have a successful outcome.
